子痫前期母亲所娩新生儿视网膜出血的影响因素分析

摘　　要：目的探讨子痫前期(PE)母亲所娩新生儿视网膜出血(RH)的影响因素。方法回顾性分析2016年2月至2020年11月在我院进行产检并已分娩的279例PE母亲及其所产新生儿临床资料。将新生儿出生后1周内RetCam眼底检查发现RH者纳入RH组,未发生RH者纳入正常组,对比两组临床资料,采用多因素logistic回归分析PE母亲所娩新生儿RH的影响因素。结果RH组83例(29.75%),正常组196例(70.25%)。RH组分娩方式、是否应用宫缩剂、是否脐带绕颈、新生儿是否窒息、产妇血小板计数(PLT)、产妇平均血小板容积(MPV)、产妇白蛋白(Alb)指标与正常组比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析:阴道分娩、应用宫缩剂、脐带绕颈、新生儿窒息、PLT、MPV、Alb均是PE母亲所娩新生儿RH的影响因素(P<0.05)。结论PE母亲所娩新生儿RH的影响因素有阴道分娩、应用宫缩剂、脐带绕颈、新生儿窒息及产妇PLT、MPV、Alb。临床上应针对各诱因,积极干预,降低新生儿RH发生率。Objective To explore the influencing factors of neonatal retinal hemorrhage(RH)delivered by mothers with preeclampsia(PE).Methods A retrospective analysis of the clinical data of 279 PE mothers and their newborns who had undergone prenatal check-ups in our hospital from February 2016 to November 2020.Include the newborns with RH found in RetCam’s fundus examination within 1 week after birth into the RH group,No RH was included in the normal group,Compare the clinical data of the two groups,Multivariate logistic regression analysis was used to find out the influencing factors of RH in newborns delivered by PE mothers.Results There were 83 cases(29.75%)in the RH group and196 cases(70.25%)in the normal group.The RH group’s delivery method,whether to use uterine contractions,whether the umbilical cord around the neck,whether the newborn is asphyxiated,maternal platelet count(PLT),maternal mean platelet volume(MPV),maternal albumin(Alb)indicators compared with the normal group,differences there is statistical significance(P<0.05).Multivariate logistic regression analysis:vaginal delivery,application of uterine contractions,umbilical cord around the neck,neonatal asphyxia,PLT,MPV,Alb are all influencing factors of RH in newborns delivered by PE mothers(P<0.05).Conclusion The influencing factors of RH in newborns delivered by PE mothers include vaginal delivery,uterine contractions,umbilical cord around the neck,neonatal asphyxia,and maternal PLT,MPV,and Alb.Clinically,each trigger should be targeted,actively intervene,reduce the incidence of neonatal RH.
