床边去白细胞滤器对血小板输注效果的影响

摘　　要：目的探讨去白细胞滤器对血小板临床输注效果的影响。方法对浙江省中医院血液科59例患者共输注706次单采血小板后的临床效果进行回顾性分析。其中,352次使用床边血小板型去白细胞滤器(去白细胞组),354次不使用床边滤器(不去白细胞组)。通过计算血小板计数增量校正值(CCI)分析床边滤器的有效性,观察输血反应发生例数,比较不同疾病类别使用或不使用滤器的有效性和安全性,综合评价床边去白细胞滤器对血小板输注的效果。结果去白细胞组的CCI值为(6.70±10.28)×10^(9)/L,输注有效率为46.0%,与不去白细胞组比较均无统计学差异(P>0.05)。去白细胞组发生非溶血性发热性输血反应(FNHTR)14例(4.0%),不去白细胞组发生FNHTR共49例(13.8%),组间比较有统计学差异(P<0.01)。再生障碍性贫血(AA)患者输注后的CCI值、输注有效率和FNHTR发生率,去白细胞组明显高于不去白细胞组(P<0.01)。结论输注单采血小板时使用床边滤器不会降低有效性,且安全性均优于不使用床边滤器。Objective To evaluate the effect of leukoreduction filter on clinical transfusion of apheresis platelets.Methods A retrospective study was conducted on the clinical effects of 706 apheresis platelets transfusion in 59 clinical cases in hematology department of Zhejiang Hospital of Traditional Chinese Medicine.352 rimes were treated with a bedside platelet filter(leukocyte-depleted group)and 354 times were not treated with a bedside filter(non-leukocyte-depleted group).The effectiveness of the bedside filter was analyzed by calculating corrected count increment(CCI)and observing the number of blood transfusion reacrions,the effectiveness and safety of using filters for different disease classes were further analyzed,so as to comprehensively evaluate the effect of bedside leukoreduction filter on platelet transfusion.Results The CCI value of the leukocyte-depleted group was(6.70±10.28)×10^(9)/L,and the inflision effective rate was 46.0%,which showed no statistical difference compared with that of the non-leukocyte-depleted group.Nonhemolytic febrile blood transfusion reaction(FNHTR)occurred in 14 patients(4.0%)in the leukocyte-depleted group and 49 patients(13.8%)in the non-leukocyte-depleted group,with statistical difference between the two groups(P<0.01).CCI value,infusion response rate and incidence of FNHTR in aplastic anemia(AA)patients after infusion were significantly higher in leukocyte-depleted group than in non-leukocyte-depleted group(P<0.01).Conclusion The use of bedside filter for transfusion of apheresis phitelets does not reduce effectiveness and is safer than that without bedside filter.
