Chief Li Peng’s Experience in Differentiating and Treating Compensatory Menstruation

摘　　要：李澎认为代偿性月经属祖国医学“倒经”范畴,乃“火迫血行”所致,其火之缘由主要责之于肝肾功能失调,临床辨治常从调补肝肾入手,治疗时首分虚火、实火,虚火多为“龙火上奔型”,治宜补肾填精,滋阴降火,导龙入海;实火多为“肝经郁火型”,治宜疏肝清热,引血下行;另外,亦汲取张锡纯辨治该病之经验,多于辨证取穴方中加入健脾强胃之穴(足三里、中脘、公孙、内关)以镇安冲气,加止鼻衄之经验效穴(孔最、上星)以增强疗效。Professor Li Peng thinks that compensatory menstruation belongs to the category of“vicarious menstruation”in traditional Chinese medicine,induced by“fire forcing blood moving”,the cause of fire is mainly due to liver and kidney dysfunction,clinical syndrome differentiation and treatment often starts with regulating and tonifying the liver and kidney,in the treatment,the first point of treatment is deficiency fire and excessive fire.Deficiency fire is of mostly“dragon fire running upward”pattern,therapeutic principle should tonify the kidney and fill essence,nourish Yin and reduce fire,guide fire into kidney;excessive fire is of“stagnant fire in liver meridian”pattern,therapeutic principle should soothe the liver,clear away heat,draw blood downward;in addition,by absorbing Zhang Xichun’s experience in differentiating and treating the disease,he adds the acupoints of strengthening spleen and stomach including Zusanli(ST36),Zhongwan(RN12),Gongsun(SP4)and Neiguan(PC6)in the prescription of syndrome differentiation to calm down the flush Qi,and the effective acupoints including Kongzui(LU6)and Shangxing(GV23,DU23)of stopping epistaxis to enhance the curative effects.
