心衰患者心电图QRS波时限的特征及其与心功能的相关性

摘　　要：目的:研究心衰患者心电图QRS波时限的特征及其与心功能的相关性。方法:选择我院的120例心衰患者为心衰组。心衰组被进一步分为收缩性心衰(SHF)组(54例)和舒张性心衰(DHF)组(66例)。另选择同期健康体检者59例为健康对照组。比较各组心电图QRS波时限、LVEF、体循环血管阻力(SVR)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV),并分析QRS波时限与心功能指标的相关性。结果:与健康对照组比较,心衰组QRS波时限[(80.58±3.20)ms比(123.92±9.78)ms]、SVR[(122.03±7.80)ml/min比(156.24±12.41)ml/min]、LVEDV[(116.47±7.71)ml比(143.21±11.97)ml]及LVESV[(49.31±6.82)ml比(76.49±6.46)ml]均显著升高,LVEF[(66.21±4.95)%比(48.71±3.49)%]显著降低,P均=0.001。与DHF组比较,SHF组QRS波时限[(104.75±4.78)ms比(142.08±5.50)ms]、SVR[(135.46±11.17)ml/min比(161.23±19.09)ml/min]、LVEDV[(125.54±9.43)ml比(152.31±12.21)ml]及LVESV[(71.65±5.25)ml比(80.83±6.29)ml]均显著升高,LVEF[(59.96±8.06)%比(38.89±6.85)%]显著降低,P均=0.001。Pearson相关性分析显示,心电图QRS波时限与LVEF呈显著负相关(r=-0.693,P=0.001),与SVR、LVEDV及LVESV呈显著正相关(r=0.609~0.735,P均=0.001)。结论:心衰患者心电图QRS波时限较长,且其与心衰患者心功能显著相关。Objective:To study features of electrocardiogram(ECG)QRS duration in patients with heart failure(HF)and its correlation with cardiac function.Methods:A total of 120 HF patients of our hospital were selected as HF group.HF group was further divided into systolic heart failure(SHF)group(n=54)and diastolic heart failure(DHF)group(n=66).Another 59 healthy volunteers in same term were regarded as healthy control group.ECG QRS duration,LVEF,systemic vascular resistance(SVR),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV)were compared among all groups.Correlation between QRS duration and cardiac function indexes were analyzed.Results:Compared with healthy control group,there were significant rise in QRS duration[(80.58±3.20)ms vs.(123.92±9.78)ms],SVR[(122.03±7.80)ml/min vs.(156.24±12.41)ml/min],LVEDV[(116.47±7.71)ml vs.(143.21±11.97)ml]and LVESV[(49.31±6.82)ml vs.(76.49±6.46)ml],and significant reduction in LVEF[(66.21±4.95)%vs.(48.71±3.49)%]in HF group,P=0.001 all.Compared with DHF group,there were significant rise in QRS duration[(104.75±4.78)ms vs.(142.08±5.50)ms],SVR[(135.46±11.17)ml/min vs.(161.23±19.09)ml/min],LVEDV[(125.54±9.43)ml vs.(152.31±12.21)ml]and LVESV[(71.65±5.25)ml vs.(80.83±6.29)ml],and significant reduction in LVEF[(59.96±8.06)%vs.(38.89±6.85)%]in SHF group,P=0.001 all.Pearson correlation analysis indicated that ECG QRS duration was significant inversely correlated with LVEF(r=-0.693,P=0.001),and significant positively correlated with SVR,LVEDV and LVESV(r=0.609~0.735,P=0.001 all).Conclusion:ECG QRS duration is significantly longer and it is significantly correlated with cardiac function in HF patients.
