小儿腹腔镜美克尔憩室术后早期活动量化及效果评价

摘　　要：目的基于ERAS理念,探讨小儿腹腔镜美克尔憩室术后早期活动的量化及其应用效果。方法选择南京医科大学附属无锡儿童医院2019年4月—年12月期间腹腔镜美克尔憩室术后患儿25例为对照组,2020年1月—9月期间腹腔镜美克尔憩室术后患儿25例为观察组。对照组护士按护理常规指导患儿术后下床活动,观察组在对照组干预措施的基础上应用快速康复理念对患儿早期下床活动进行护理干预,比较分析2组患儿运动量、首次排气时间、术后并发症、住院天数及患儿依从性。结果观察组术后步行锻炼活动量高于对照组,术后首次排气时间早于对照组,术后住院天数低于对照组,差异均有统计学意义(P<0.05)。2组患儿术后并发症及术后早期活动依从性比较差异无统计学意义(P>0.05),但观察组步行锻炼依从性高于对照组,差异具有统计学意义(P<0.05)。结论基于ERAS理念的早期活动在小儿腹腔镜美克尔憩室术后能促进患儿术后排气,缩短住院时间,并且患儿依从性较高,可行性较好。Objective To investigate the effect of early activity based on ERAS concept in pediatric laparoscopic Meckel diverticulum operation.Methods A total of 25 children with laparoscopic Meckel's diverticulum operation were selected as the control group from April 2019 to September 2019,and 25 children with laparoscopic Meckel's diverticulum operation from October 2019 to March 2020 were selected as the intervention group.The nurses in the control group guided the children to do exercises after surgery according to the nursing routine,and the intervention group applied the ERAS concept to the children's early exercises on the basis of the intervention measures in the control group.The time of the first exhaust,postoperative complications and length of hospital stay were compared and analyzed between the two groups.The starting time of each activity and the number of walking steps were recorded by self-made table.Results The amount of postoperative walking exercise in the intervention group was higher than that in the control group,the first postoperative exhaust time was earlier than that in the control group,and the postoperative hospital stay in the intervention group was lower than that in the control group,with statistical significance(P<0.05).There was no significant difference in postoperative complications and compliance between the two groups(P>0.05),but the compliance of walking exercise in the intervention group was higher than that in the control group,with statistical significance(P<0.05).Conclusion Early activities based on ERAS can promote postoperative exhaust and shorten the length of hospital stay in children after laparoscopic Meckel diverticulum surgery,and the compliance of children is high and the feasibility is good.
