黄芪桂枝五物汤加减在糖尿病周围神经病变患者中的应用效果研究

摘　　要：目的:探究黄芪桂枝五物汤加减在糖尿病周围神经病变患者中的应用效果。方法:选取2020年1月至2021年1月于我院医治的糖尿病周围神经病变患者80例为研究对象,依据随机数字表法将其分为对照组和试验组,每组各40例。两组患者入院后进行常规治疗,对照组给予贝前列素钠片、依帕司他片,试验组在此基础上施以黄芪桂枝五物汤加减治疗,两组患者均连续治疗3个月。比较两组患者治疗前后的中医症状评分、血清炎性因子水平和神经传导速度变化。结果:与治疗前相比,两组患者治疗后手足麻木、肢体末端疼痛、神疲乏力、气短懒言、自汗畏风、肌肤甲错等中医症状评分,血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均降低(P<0.05),且试验组降低较对照组更为明显(P<0.05);治疗后两组患者运动神经传导速度(MCV)、感觉神经传导速度(SCV)均加快(P<0.05),且试验组加快较对照组更为明显(P<0.05)。结论:黄芪桂枝五物汤加减治疗能有效加快糖尿病周围神经病变患者神经传导速度,改善中医症状,降低血清炎性因子水平,促进病情好转。Objective:To explore the efficacy of huangqi guizhi wuwu decoction in the treatment of patients with diabetic peripheral neuropathy.Methods:80 patients with diabetic peripheral neuropathy who were treated in our hospital from January 2020 to January 2021 were selected as the research objects.According to the random number table method,they were divided into the control group and the test group,with 40 cases in each group.Both two groups of patients received conventional treatment after admission.The control group was given beraprost sodium tablets and epalrestat tablets,and the test group was given huangqi guizhi wuwu decoction based on the treatment of the control group.Both groups were treated for 3 months continuously.The TCM symptom scores,serum inflammatory factor levels and nerve conduction velocity before and after the treatment were compared between the two groups.Results:After treatment,the TCM symptoms scores on numbness of hands and feet,pain at the end of the limbs,fatigue,shortness of breath,spontaneous sweating,fear of wind,squamous and dry skin,the level of serum interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)of the two groups of patients decreased compared with those before the treatment(P<0.05),and the decrease in the test group was more obvious than that in the control group(P<0.05).After treatment,the motor nerve conduction velocity(MCV)and sensory nerve conduction velocity(SCV)of the two groups of patients accelerated(P<0.05),and the acceleration of the test group was more obvious than that of the control group(P<0.05).Conclusion:The treatment of huangqi guizhi wuwu decoction can effectively accelerate the nerve conduction velocity of patients with diabetic peripheral neuropathy,improve TCM symptoms,reduce the serum inflammatory factor level,and promote the improvement of the condition.
