中青年肺真菌病患者生活质量及影响因素研究

摘　　要：目的调查中青年肺真菌病患者生活质量现状,分析影响生活质量的因素,为制订干预措施提供临床依据。方法选取2017年10月—2020年4月就诊于南京市某三级甲等医院的221例中青年肺真菌病患者为研究对象,采用一般资料调查表、广泛性焦虑量表(GAD-7)、病人健康问卷抑郁量表(PHQ-9)、世界卫生组织生活质量调查简表(WHOQOL-BREF)进行调查。结果中青年肺真菌病患者生活质量总得分为59.93±8.12分(P<0.05);多因素回归分析显示,现居住地、家庭人均月收入、病程(月)、症状(种)、合并疾病(种)、接触史(家禽、宠物、粉尘等)、药物不良反应,是影响患者生活质量的重要因素(P<0.05),患者生活质量与负性情绪呈负相关(P<0.01)。结论中青年肺真菌病患者生活质量普遍偏低,影响因素较多,负性情绪与生活质量密切相关,提示今后工作应该重视中青年肺真菌病患者,专业评估,采取针对性干预措施,提高生活质量。Objective To investigate the current life quality of young and middle-aged patients with pneumomycosis and analyze the factors that affect the quality of life,and in order to provide a clinical basis for the formulation of intervention measures.Methods 221 young and middle-aged patients with pneumomycosis who were treated in a tertiary hospital in Nanjing from October 2017 to April 2020 were selected as the research objects.General information questionnaire,Generalized Anxiety Disorder-7(GAD-7)scale,Patient Health Questionnaire for Depression(PHQ-9),and WHOQOL-BREF were used for the survey.Results The total quality of life was 59.93±8.12(P<0.05)in young and middle-aged patients with pneumomycosis.Multivariate regression analysis showed that current residence,family monthly income per capita,course of disease,symptoms,combined disease,exposure history(poultry,pets,dust,etc.),and adverse drug reactions were the important factors affecting patients’quality of life(P<0.05).Patients’quality of life was negatively correlated with negative emotions(P<0.01).Conclusion The quality of life of young and middle-aged patients with pneumomycosis is generally low,with many influencing factors.Negative emotions are closely related to the quality of life,suggesting that more attention should be paid to young and middle-aged patients with pneumomycosis in future work.Professional assessment and targeted intervention measures should be taken to improve the quality of life.
