摘 要:目的观察气管植入间充质干细胞(MSCs)联合部分液体通气,对海水淹溺致肺损伤大鼠治疗作用。方法大鼠分为CONTROL组(C组)、MODEL组(M组)、REGULAR组(R组)、PFC组(P组)和PFC+MSC(PM组)组。除C组外,各组大鼠气管切开后注入海水构建肺损伤模型。R组给予常规机械通气,P组给予部分液体通气,PM组给予气管内植入MSC后,予以部分液体通气。留取造模后1h、3h动脉血气,观察肺组织大体形态及病理学检查,ELISA检测血清中TNF-α、IL-1β、IL-6、IL-10含量变化,激光共聚焦显微镜观察PM组大鼠肺组织内MSCs。结果 P组和PM组较R组较快的改善动脉血氧分压及氧合指数,并明显减轻海水注入导致肺组织损伤改变。PM组较R组TNF-α和IL-6水平明显降低,较P组IL-6水平明显降低,与R组、P组分别比较,IL-10水平升高,差异有统计学差异(P<0.01))。激光共聚焦显微镜观察可见PM组肺组织有红色荧光标记的MSCs。结论气管植入间充质干细胞联合部分液体通气,可减轻海水淹溺致肺组织损伤,改善缺氧,降低与损伤相关炎症因子TNF-α、IL-1β、IL-6水平,探索了治疗海水淹溺致肺损伤新的治疗手段。Objective To observe the effect of airway injection of mesenchymal stem cells(MSCs) combined with partial liquid ventilation on lung injury in rats induced by seawater drowning. Methods Rats were divided into CONTROL group(C group), MODEL group(M group), REGULAR group(R group), PFC group(P group) and PFC + MSC group(PM group). Except the group C, rats in each group were tracheotomized and in-jected with seawater to establish lung injury model. Group R was given conventional mechanical ventilation, group Pwas given partial liquid ventilation, and group PM was given partial liquid ventilation after tracheal injection of MSCs.Arterial blood gas was collected at 1 hour and 3 hours after modeling. The morphology and pathological examination oflung tissues were observed. The levels of TNF-α, IL-1 β, IL-6, and IL-10 in serum were detected by ELISA. TheMSCs in the lung tissues of PM group were observed by confocal laser scanning.Results Compared with Group R,P group and PM group improved PaO2 and oxygenation index, and significantly reduced lung tissue damage caused byseawater. The levels of TNF-α and IL-6 in group PM were significantly lower than those in group R. The levels of IL-6 in group PM were significantly lower than that in group P(P< 0. 01), and the level of IL-10 in group PM washigher than that in group R and group P, respectively(P< 0. 01). Red fluorescence-labeled MSCs were observed inPM group under confocal laser scanning.Conclusion Tracheal implantation of MSCs combined with partial liquidventilation can alleviate lung tissue injury, improve hypoxia, and reduce the levels of injury-related inflammatory in-cluding TNF-α, IL-1β, and IL-6 in rats with lung injury induced by seawater drowning, which explores a new treat-ment strategy for seawater drowning-induced lung injury.
