摘 要:目的探讨哮喘患者外周血利钠肽受体A(NPRA)、GATA结合蛋白3(GATA 3)mRNA的表达与免疫功能变化及病情严重程度的关系。方法选取武汉市武昌医院收治的哮喘患者156例(哮喘组)、正常体检志愿者60例作为对照组,检测外周血中NPRA mRNA、GATA 3 mRNA表达、调节性T细胞(Treg)、Th1细胞、Th2细胞、细胞因子水平。结果对照组的NPRA mRNA、GATA 3 mRNA表达水平低于哮喘组急性发作期患者、稳定期患者(P<0.05);稳定期哮喘患者的NPRA mRNA、GATA 3 mRNA表达水平低于哮喘组急性发作期患者(P<0.05);外周血的Treg细胞、Th1细胞、Th2细胞、IL-2、IFN-γ水平比较,对照组高于哮喘组急性发作期患者、稳定期患者(P<0.05);IL-6、TNF-α水平组间比较,对照组均低于哮喘组急性发作期患者、稳定期患者(P<0.05);NPRA mRNA、GATA 3 mRNA表达与IL-2、IFN-γ水平呈显著负相关性(P<0.05);NPRA mRNA、GATA 3 mRNA表达与IL-6呈正相关性(P<0.05)。结论哮喘患者外周血中NPRA mRNA、GATA 3 mRNA表达上调,可能与外周血T细胞介导的免疫细胞因子失衡有关。Objective To explore the relationship between the expression of peripheral blood natriuretic peptide receptor A(NPRA) and GATA binding protein 3(GATA 3) mRNA in patients with asthma and the change of immune function and the severity of the disease. Methods 156 asthmatic patients(asthma group) and 60 normal physical examination volunteers who were treated in Wuhan Wuchang Hospital were selected. The levels of NPRA mRNA, GATA 3 mRNA expression, regulatory T cells(Treg), Th1 cells, Th2 cells, and cytokine levels in peripheral blood were detected. Results The levels of NPRA mRNA and GATA 3 mRNA in the control patients were lower than those in the patients with acute asthma and stable asthma groups(P<0.05);The expression levels of NPRA mRNA and GATA 3 mRNA in patients with stable asthma were lower than those in patients with acute attack asthma(P<0.05);The levels of Treg cells, Th1 cells, Th2 cells, IL-2, IFN-γ in the control group were higher than those in the patients with acute asthma and stable asthma groups(P<0.05). In terms of IL-6 and TNF-α, patients in the control group were significantly lower than in the patients with asthma group in acute attack and stable asthma groups(P<0.05);The mRNA expression of NPRA and GATA 3 was significantly negatively correlated with IL-2 and IFN-γ levels(P<0.05) but positively correlated with IL-6(P<0.05). Conclusion The up-regulation of peripheral blood NPRA mRNA and GATA 3 mRNA in asthmatic patients may be related to peripheral blood T cell-mediated immune cytokines imbalance.
