复明片联合人工泪液治疗角膜移植术后干眼症临床疗效及对生活质量的影响

摘　　要：目的观察复明片联合人工泪液治疗角膜移植术后干眼症患者的临床疗效及对生活质量的影响。方法将2017年9月—2019年9月河北医科大学第三医院诊治的100例角膜移植术后干眼症患者随机分为2组,每组50例。对照组给予单独人工泪液治疗,治疗组在对照组治疗基础上给予复明片口服12周。观察2组治疗前及治疗4周、8周、12周泪膜破裂时间、泪液分泌试验评分、泪河高度评分、角膜荧光染色评分、临床症状评分和眼表疾病指数评分,治疗12周后评价2组患者的临床疗效。结果2组患者的泪膜破裂时间、基础泪液分泌量、泪河高度评分随着治疗时间的推移而逐渐增加(P均<0.05),且治疗组治疗后各时间点均明显高于对照组(P均<0.05);2组患者的角膜荧光染色评分、临床症状评分、眼表疾病指数评分随着时间的推移而逐渐降低(P均<0.05),且治疗组治疗后各时间点均明显低于对照组(P均<0.05)。治疗组总有效率为76%(38/50),对照组为60%(30/50),治疗组明显高于对照组(P<0.05)。结论复明片联合人工泪液可显著改善角膜移植术后干眼症患者的临床症状、泪液分泌量、泪液稳定性及角膜病损情况,还可明显提高患者生活质量,其临床疗效优于单纯人工泪液治疗,并随着治疗时间延长,治疗优势越明显。Objective It is to observe the clinical efficacy of Fuming tablets combined with artificial tears in the treatment of patients with dry eye after keratoplasty and its effect on the quality of life.Methods One hundred patients with dry eye after corneal transplantation diagnosed and treated in the Third Hospital of Hebei Medical University from September 2017 to September 2019 were randomly divided into 2 groups,with 50 cases in each group.The control group was given artificial tears alone,and the treatment group was treated with Fuming tablets orally for 12 weeks on the basis of treatment in the control group.The tear film rupture time,tear secretion test score,tear river height score,corneal fluorescence staining score,clinical symptom score and ocular surface disease index score were observed before treatment and after 4 weeks,8 weeks,and 12 weeks of treatment in the two groups,and the clinical efficacy of the two groups was evaluated after 12 weeks of treatment.Results The tear film rupture time,basic tear secretion,and tear river height scores of the two groups were gradually increased with the passage of treatment time(P<0.05),and the treatment group was significantly higher than the control group at each time point after treatment(all P<0.05);The corneal fluorescence staining score,clinical symptom score,and ocular surface disease index score of the two groups were gradually decreased with the passage of treatment time(all P<0.05),and the treatment group was significantly lower than the control group at all time points after treatment(all P<0.05).The total effective rate in the treatment group was 76%(38/50),and that in the control group was 60%(30/50),the treatment group was significantly higher than the control group(P<0.05).Conclusion Fuming tablets combined with artificial tears can significantly improve the clinical symptoms,tear secretion,tear stability and corneal damage in patients with dry eye after keratoplasty,and can also significantly improve the quality of life of patients,and its clinical eff
