摘　　要：目的:探讨通督治郁针法联合头针疗法治疗卒中后抑郁(PSD)的疗效及对血清5-HT和BDNF水平影响。方法:选取本院收治的卒中后抑郁患者86例,采用动态随机化原则分为对照组(头针疗法43例)和治疗组(通督治郁针法联合头针疗法43例),对比治疗后的临床疗效、中医症候积分、HAMD评分、NIHSS评分、Barthel指数评分及血清5-HT和BDNF水平。结果:经过治疗后,治疗组临床疗效总有效率明显较高(P<0.05);治疗后两组血清5-HT和BDNF水平及Barthel指数评分明显升高,中医症候积分、HAMD评分和NIHSS评分明显降低(P<0.05),且治疗组改善较多(P<0.05)。结论:PSD患者采用通督治郁针法联合头针治疗具有较好的疗效。Objective:To investigate the effect of Tongdu Zhiyu acupuncture combined with scalp acupuncture in the treatment of post-stroke depression(PSD)and its influence to serum levels of 5-HT and BDNF.Methods:A total of 86 patients with post-stroke depression were randomly divided into the control group(n=43)and the treatment group(n=43)by dynamic randomization principle.The control group was treated with scalp acupuncture;on which basis,the treatment group was also treated with Tongdu Zhiyu acupuncture.Clinical efficacy,TCM syndrome score,HAMD score,NIHSS score,Barthel index score and serum levels of 5-HT and BDNF were compared between the two groups after the treatment.Results:The total clinical efficacy was significantly higher in the treatment group than that in the control group(P<0.05).The serum levels of 5-HT and BDNF,as well as Barthel index score were significantly increased,whereas TCM syndrome score,HAMD score and NIHSS score were significantly decreased in the two groups after the treatment(P<0.05);of which the improvements in the treatment group were more significant(P<0.05).Conclusion:Tongdu Zhiyu acupuncture combined with scalp acupuncture has a better curative effect in the treatment of PSD.
