摘  要:目的了解中山市2018年呼吸道合胞病毒(Respiratory Syncytial Virus, RSV)流行株序列特点分析;为分析类似疫情中所检出的RSV进行快速分型和溯源提供依据。方法使用基因扩增和生物信息学分析方法,对2018年流感样病例监测样及一起社区聚集性流感样疫情样本的RSV阳性病毒进行亚型分型和G基因高变区序列特征分析。结果在1 025例流感样病例的标本共检出60份RSV核酸阳性,RSV阳性率为5.8%;经过PCR和毛细管凝胶电泳分析,获得的6例标本和1例疫情标本中RSV的基因分型均为RSV A亚型;7例RSV阳性标本的G蛋白基因扩增成功并且测序成功(ZS2018RSV01-07,Genbank MW455129-MW455135);与监测样本相比,疫情标本在高变区T264A,S313F,T319K等位点存在特异性氨基酸位点突变。结论本研究获得的中山市2018年RSV的流行株为A亚型中ON1基因型,3月份社区聚集性肺炎中获得的疫情分离株RSV中G基因高变区的位点突变可能有助于改变宿主细胞对病毒的识别能力以及病毒逃避宿主的免疫反应,从而可能更加有助于RSV的爆发流行,建议加强对RSV的监测。Objective To analyze the local epidemic characteristics of respiratory syncytial virus(RSV) in Zhongshan in 2018, and to construct a local RSV phylogenetic tree and analyze the origin of RSV derived from local outbreaks. Methods Samples from patients within monitoring influenza-like cases in 2018 in Zhongshan and an unidentified pneumonia epidemic case in Zhongshan community in March 2018, detected positive with RSV, were subjected to PCR, and sequenced and analyzed by bioinformatics. Results In 1 025 cases of respiratory syndrome, 60 samples were detected positive with RSV. The positive rate was 5.8%. The genotype of 7 isolated RSV strains was subtype A. The full length of G genes of 6 RSV strains from monitoring cases(ZS2018 RSV01-06, Genbank MW455129-MW455134) and 1 RSV strain from community outbreak case(ZS2018 RSV07, Genbank MW455135) were derived. T264 A, S313 F and T319 K in the HRV2 of ZS2018 RSV07 were mutated in the HVR2(Highly variable region 2), which was different from monitoring cases. Conclusion The genotype of 7 RSV strains is ON1, which belongs to subtype A. The mutations from community outbreak case may lead the change of the recognition to virus of the host cell and help immune escape of virus. These mutations may help the outbreak of the virus and surveillance needs to be strengthened.
