双钢板治疗TKA术后股骨假体周围骨折的临床疗效

摘  要:目的:分析双钢板治疗TKA术后股骨假体周围骨折的临床疗效。方法:回顾我科2014年1月至2019年12月完成的15例TKA术后股骨假体周围骨折的患者,根据Rorabeck分型均为Ⅱ型,其中男3例,女12例,年龄63-76岁,平均69.2岁,所有患者均采用内、外侧双钢板固定,未进行植骨。结果:所有患者均获得随访,平均随访时间20个月。术后6w时HSS评分平均(72.6±7.2)分,6个月时(90.3±2.8)分,末次随访时平均(93.6±1.4)分。术后12个月时,所有骨折均完全愈合,未出现内固定失效,膝关节活动度较伤前稍差,活动范围基本可达到90°-115°。结论:TKA术后股骨假体周围骨折,对于假体稳定的RombeckⅡ型骨折采用内、外侧双钢板固定,无需植骨即可获得良好的临床疗效。Objective:To observe the clinical effect of the double plates fixation for treatment of the patients with periprosthetic fractures after TKA.Methods:From January 2014 to December 2019,a total of 15 patients happened periprosthetic fractures after the surgery of total knee arthroplasty(TKA),and all were diagnosed as type Ⅱ according lo the Roraheck classification,including 3 male and 12 female,aged between 63 and 76 years,with an average age of 69.2 years.All patients were treated with double plates on the both sides without bone grafting.Results:All patients were followed up with an average time of 20 months.The average HSS score at 6 weeks,6 months and the last follow-up after surgery were(72.6±7.2)points,(90.3±2.8)points and(93.6±1.4)points,respectively.At 12 months after the operation,all fractures were completely healed without internal fixation failure,and the range of motion of the knee joint was slightly worse than before the injury,and the range of motion could basically reach 90°-115°.Conclusion:For the stable Roraheck type Ⅱ fractures,double plates fixation without bone grafting can obtain a good clinical result in patients with periprosthetic fractures after TKA.
