应用远端锁定钢板治疗移位的锁骨胸骨端骨折的疗效

摘　　要：目的探讨应用远端解剖型锁定钢板治疗完全移位的锁骨胸骨端骨折的方法及疗效。方法自2015年3月至2019年6月在我科采用远端解剖型锁定钢板倒置治疗完全移位的锁骨胸骨端骨折患者11例,根据Edinburgh分型:B1型6例,B2型5例。术后定期随访,记录患者术后并发症、骨折愈合时间、末次平均DASH得分进行功能评价。结果术后所有患者获得随访,时间为10〜16个月,平均12.6个月,均获得骨性愈合,愈合时间为8〜20周,平均12.5周,未见关节僵硬、内固定松动、感染、创伤性关节炎等远期并发症。末次平均DASH得分为1.8(0〜12.1);功能评分:优8例,良3例。所有患者均对手术结果满意。9例患者恢复以前的工作岗位。结论采用远端解剖型锁定钢板倒置固定治疗完全移位的锁骨胸骨端骨折,可以维持骨折稳定,促进骨折愈合,早期获得良好的功能恢复,是一种可选择的治疗手段。Objective To investigate the method and clinical efficacy of distal anatomical locking plate in the treatment of completely displaced fractures of sternal end of clavicle.Methods From March 2015 to June 2019,11 patients with completely displaced fractures of sternal end of clavicle were treated with distal anatomical locking plate inversion in our department.According to Edinburgh classification,there were 6 cases of type B1 and 5 cases of type B2.The patients were followed up regularly after operation.The postoperative complications,fracture healing time and the last average DASH score were recorded for functional evaluation.Results All the patients were follow-up for 10 to 16 months,with an average of 12.6 months.The bone healing was achieved in all the cases.The healing time was 8 to 20 weeks,with an average of 12.5 weeks.There were no long-term complications such as joint stiffness,internal fixation loosening,infection and traumatic arthritis.The last average DASH score was 1.8(ranged,0 to 12.1).The functional score was excellent in 8 cases and good in 3 cases.All the patients were satisfied with the surgical results.Nine patients returned to their previous jobs.Conclusion The treatment of completely displaced fractures of sternal end of clavicle with inverted distal anatomical locking plate can maintain fracture stability,promote fracture healing and obtain good functional recovery in the early stage.It is an alternative treatment.
