摘 要:目的研究高分辨率超声检查在临床诊断肩袖损伤中的作用。方法自2020年1月至12月我们对49例患者进行高分辨率超声检查(其中15例行肩关节镜手术),对53例患者术前进行核磁共振(magnetic resonance imaging,MRI)检查。影像学检查结果与关节镜检查结果进行比较。结果比较敏感性、特异性、阳性预测值和阴性预测值后,我们发现高分辨率超声和MRI在检测各种类型的肩袖损伤或肩袖部分撕裂差异均无统计学意义。高分辨率超声在检测肩袖完全性撕裂时,比MRI更具敏感度(P<0.05),但在其他参数上差异均无统计学意义。结论高分辨率超声和MRI对各种类型肩袖损伤的诊断效力相当。高分辨率超声在诊断肩袖完全性撕裂时较MRI更具敏感度,可作为诊断肩袖损伤的首选方法。Objective To study the role of high-resolution ultrasonography in the clinical diagnosis of rotator cuff injury.Methods From January to December 2020,49 patients underwent high-resolution ultrasonography(15 patients underwent arthroscopic surgery)and 53 patients underwent magnetic resonance imaging(MRI)before operation.The imaging results were compared with those of arthroscopy.Results After comparing the sensitivity,specificity,positive predictive value and negative predictive value,we found that there was no significant difference between high-resolution ultrasonography and MRI in the detection of various types of rotator cuff injury or partial thickness tears.High resolution ultrasonography was more sensitive than MRI in detecting complete rotator cuff tears(P<0.05),hut there was no significant difference in other parameters.Conclusion High resolution ultrasonography and MRI are equally effective in the diagnosis of rotator cuff injuries.High resolution ultrasonography is more sensitive than MRI in the diagnosis of complete rotator cuff tears.It can be the first choice for the diagnosis of rotator cuff injury.
