改良Stoppa入路在骨盆与髋臼骨折治疗中的临床应用分析

摘　　要：目的探讨对骨盆与髋臼骨折手术治疗采用改良Stoppa入路的临床效果。方法方便选取该院2016年1月—2020年4月收治的40例改良Stoppa入路切开复位治疗骨盆与髋臼骨折患者为观察组,纳入同期40例行髂腹股沟入路切开复位治疗骨盆与髋臼骨折患者为对照组,比较两组患者术后1周骨折复位情况及术后3个月关节功能恢复情况,统计术后并发症发生率。结果观察组手术时间更少,术中出血量更少,而住院时间也更短,差异有统计学意义(t=4.939、11.892、11.655,P<0.001);观察组骨折复位优良率为92.50%,对照组仅为75.00%,差异有统计学意义(χ^(2)=4.501,P=0.034);观察组患者髋关节功能优良率为92.50%对照组仅为70.00%,差异有统计学意义(χ^(2)=6.646,P=0.010);观察组并发症发生率与对照组相比显著更低,差异有统计学意义(P<0.05)。结论骨盆并髋臼骨折切开复位内固定手术治疗中改良Stoppa入路能提高骨折复位效果和髋关节功能,减少术后并发症,临床价值显著。Objective To investigate the clinical effect of the modified Stoppa approach in the surgical treatment of pelvic and acetabular fractures.Methods 40 patients with modified Stoppa approach open reduction for pelvic and acetabular fractures in the hospital from January 2016 to April 2020 were convenience selected as the observation group,and the same period,40 cases of patients with pelvic and acetabular fractures treated with iliac-groin approach open reduction were included as the control group.The fracture reduction at 1 week after the operation and the recovery of joint function at 3 months after the operation were compared between the two groups,and the incidence of postoperative complications was calculated.Results The observation group had less operation time,less intraoperative blood losser and shorter hospital stay,the difference were statistically significant(t=4.939,11.892,11.655,P<0.001).The excellent and good rate of fracture reduction in the observation group was 92.50%,and only 75.00%in the control group,the difference were statistically significant(χ^(2)=4.501,P=0.034);the excellent and good rate of hip joint function in the observation group was 92.50%and the control group was only 70.00%,the difference were statistically significant(χ^(2)=6.646,P=0.010).The complication rate of the observation group was significantly lower than that of the control group,the difference were statistically significant(P<0.05).Conclusion In the open reduction and internal fixation of pelvic and acetabular fractures,the modified Stoppa approach can improve the fracture reduction effect and hip joint function,reduce postoperative complications,and has significant clinical value.
