双镜联合手术治疗声带息肉的嗓音功能变化、并发症及复发情况分析

摘　　要：目的分析双镜联合手术治疗声带息肉的嗓音功能变化、并发症及复发情况。方法方便选取该院2019年6月—2020年4月收治的60例声带息肉患者,经随机数表法将其分成对照组与观察组,各30例。对照组选择常规支撑喉镜手术治疗,观察组则选择支撑喉镜联合显微镜手术治疗。比较临床疗效。结果与术前相比,两组术后各项嗓音学指标(振幅微扰、基频微扰、标准化噪声能量)均显著改善,差异有统计学意义(P<0.05),观察组术后各项嗓音学指标改善情况显著优于对照组,差异有统计学意义(P<0.05)。与术前相比,两组术后声门下压(SGP)明显降低,最长发声时间(MPT)明显增加,差异有统计学意义(P<0.05);观察组术后SPG明显低于对照组,MPT明显高于对照组,差异有统计学意义(P<0.05)。观察组的并发症发生率、术后复发率分别为3.33%、0.00%,均明显低于对照组的26.67%、20.00%,比较差异有统计学意义(χ^(2)=4.706、4.630,P=0.030、0.031)。结论采用双镜联合手术对声带息肉患者进行治疗,能对其嗓音功能进行显著改善,降低声门下压,延长最长发声时间,让并发症和术后复发明显减少,具有较高的安全性,值得临床推广。Objective To analyze the changes in voice function,complications and recurrence of vocal cord polyps treated with double mirrors combined surgery.Methods 60 patients with vocal cord polyps treated in the hospital from June 2019 to April 2020 were convenience selected.They were divided into two groups by random number method,30 cases each in the control group and the observation group,the control group chose conventional support for laryngoscope surgery,the observation group chose support laryngoscope combined with microscope surgery to observe and analyze the clinical efficacy.Results Compared with preoperatively,the postoperative phonological indexes(amplitude perturbation,fundamental frequency perturbation,standardized noise energy)of the two groups were significantly improved,the difference was statistically significant(P<0.05),and the postoperative voice of the observation group of academic indicators was significantly better than that of the control group,the difference was statistically significant(P<0.05).Compared with preoperatively,the postoperative SPG of the two groups was significantly reduced,and the MPT was significantly increased,the difference was statistically significant(P<0.05);and the postoperative SPG of the observation group was significantly lower than that of the control group,and the MPT was significantly higher than that of the control group,the difference was statistically significant(P<0.05).The complication rate and postoperative recurrence rate of the observation group were 3.33%and 0.00%respectively,which were significantly lower than those of the control group of 26.67%and 20.00%,the difference was statistically significant(χ^(2)=4.706,4.630,P=0.030,0.031).Conclusion The treatment of patients with vocal cord polyps with dual-scope combined surgery can significantly improve their voice function,reduce subglottic pressure,extend the longest vocal time,significantly reduce complications and postoperative recurrence,and have high safety It is worthy of clinical promotion.
