胎儿完全性肺静脉异位引流的产前超声诊断价值探究

摘　　要：目的探讨胎儿完全性肺静脉异位引流的产前超声诊断价值。方法便利选择该院2018年8月—2020年8月收治的20例完全性肺静脉异位引流胎儿进行回顾性分析,纳入对象均经产后超声心动图或引产尸检等证实满足胎儿完全性肺静脉异位引流标准,且有完整的产前超声诊断资料,回顾性分析他们的资料,将产前超声诊断结果与产后超声心动图或引产尸检结果进行对照,总结产前超声诊断胎儿完全性肺静脉异位引流的价值。结果20例完全性肺静脉异位引流胎儿产前超声诊断符合17例,诊断准确率为85.00%,误诊3例,误诊率为15.00%;17例产前超声诊断准确中,14例伴有复杂型先天性心脏病,包括心上型5例、心下型5例、心内型4例。结论胎儿完全性肺静脉异位引流应用产前超声诊断有不错的价值,诊断准确率更高,且有特征性心动图表现,可为临床诊断与评估胎儿的病情,以及判断有合并症提供可靠的依据,值得在临床诊断中应用。Objective To investigate the value of prenatal ultrasound diagnosis of fetal complete pulmonary venous drainage.Methods Conveniently select 20 fetuses with complete pulmonary venous drainage admitted in the hospital from August 2018 to August 2020 were selected for retrospective analysis.The included subjects were confirmed by postpartum echocardiography or induced labor autopsy to meet the requirements of complete pulmonary venous ectopic fetuses drainage standards,and complete prenatal ultrasound diagnosis data,retrospectively analyze their data,compare the prenatal ultrasound diagnosis results with postpartum echocardiography or induced labor autopsy results,and summarize the prenatal ultrasound diagnosis of complete pulmonary venous drainage of the fetus the value of.Results The prenatal ultrasound diagnosis of 20 fetuses with complete pulmonary venous drainage was consistent with 17 cases,the diagnosis accuracy rate was 85.00%,3 cases were misdiagnosed,and the misdiagnosis rate was 15.00%.Of the 17 cases,the prenatal ultrasound diagnosis was accurate,and 14 cases were complicated type congenital heart disease,including 5 cases of supracardia,5 cases of subcardiac,and 4 cases of intracardiac.Conclusion Prenatal ultrasound diagnosis of complete pulmonary venous drainage of the fetus is of good value,with higher diagnostic accuracy,and characteristic cardiogram performance,which can provide reliable clinical diagnosis and evaluation of the fetal condition,as well as the judgment of comorbidities.It is worthy of application in clinical diagnosis.
