摘　　要：目的观察IOL Master700指导年龄相关性白内障合并角膜散光患者行陡峭轴切口超声乳化术的可行性。方法前瞻性病例研究。收集蚌埠医学院第一附属医院2019年6月至2020年9月78例(80只眼)年龄相关性白内障合并角膜散光行陡峭轴切口超声乳化术患者的临床资料,使用IOL Master700生物测量仪进行眼部参数的测量,分别测量患者术前及术后1周、1个月、3个月的视力、角膜散光及眼前节变化。结果(1)术后各时间段角膜平坦轴屈光力的变化甚微(P>0.05),角膜陡峭轴屈光力术后逐渐降低(P<0.05),并在术后1个月降至最小。(2)角膜散光值在术后各时间段均低于术前(P<0.05),术后1个月时降低幅度最大。(3)术后散光轴位一直维持术前状态,无明显变化(P>0.05)。(4)术后各时间段裸眼视力、最佳矫正视力均较术前明显提高(P<0.05)。(5)术后前房深度变深(P<0.05),并保持稳定。(6)术后1周及术后1个月的中央角膜厚度均较术前增厚(P<0.05),术后3个月恢复至术前厚度(P>0.05)。结论IOL Master700指导陡峭轴切口超声乳化术能够减轻患者的角膜散光值,散光轴向稳定,提高术后裸眼视力,安全有效。Objective To observe the feasibility of IOL Master700 in guiding senile cataract patients with corneal astigmatism to undergo steep axial incision phacoemulsification.Methods The clinical data of 78 age-related cataract patients(80 eyes)with astigmatism underwent steep axial incision phacoemulsification at the first Affiliated Hospital of Bengbu Medical College from June 2019 to September 2020 were collected.All patients were systematically examined with IOL Master700,and the visual acuity,corneal astigmatism and anterior segment parameters were measured before and at 1 week,1 month and 3 months after the surgery.Results There was little change in the refractive power of the corneal flat axis at each time period(P>0.05),while the refractive power of the corneal steep axis decreased gradually after the operation(P<0.05),and reached the minimum at one month after the operation.Corneal astigmatism value at all time periods after the operation were lower than those before the operation(P<0.05),and the decrease was the largest at 1 month after the operation.The postoperative astigmatism axis position maintained the preoperative state(P>0.05).The uncorrected and best corrected visual acuity at each time point after the operation were significantly better than those before the operation(P<0.05).Postoperative anterior chamber depth became deeper(P<0.05)and remained stable.The central corneal thickness increased at 1 week and 1 month after the operation(P<0.05),and recovered to the preoperative status at 3 months after the operation(P>0.05).Conclusions The steep-axis phacoemulsification guided by IOL MASTER700 can reduce the corneal astigmatism of patients,stabilize the astigmatism axis,and improve postoperative uncorrected visual acuity,which is safe and effective.
