3种Tecnis人工晶状体植入术后患者视觉质量对比研究

摘　　要：目的比较Tecnis 3种人工晶状体(ZXR00、ZMB00、ZCB00)植入术后3个月的视觉质量。方法前瞻性病例对照研究。30例年龄相关性白内障患者作为研究对象分3组,每组10只眼,植入ZXR00人工晶状体者为A组,植入ZMB00人工晶状体者为B组,植入ZCB00人工晶状体者为C组,采集A、B组术后3个月离焦曲线,应用Topcon KR-1W视觉分析仪采集术后3个月3组患者5 mm瞳孔下的调制传递函数(MTF)值并对数据进行统计学分析。结果术后3个月的离焦曲线中,A、B组在0D(5 m)、-2.5 D~-4 D(25~40 cm)处无明显差异,在-1 D~-2 D(50 cm~1 m)A组视力好于B组。术后3个月的调制传递函数(MTF)在15 c/d、75 c/d时,MTF值3组间无统计学意义(P>0.05),30 c/d、45 c/d下A、C组间MTF值无统计学意义,与B组比较有统计学意义(P<0.05),在60 c/d时A、B组间无统计学意义(P>0.05),C组与A、B组比较有统计学意义(P<0.05)。结论ZXR00较ZMB00能给患者提供更好的全程视力及视觉质量,但在暗环境下视远视觉质量较单焦晶状体仍有不足。Objective To compare the visual quality of three kinds of Tecnis intraocular lenses(ZXR00,ZMB00,ZCB00)implantation at three months after the surgery.Methods Thirty patients with age-related cataract were enrolled and divided into three groups,with 10 eyes in each group.Group A was implanted with the ZXR00 intraocular lens,group B with the ZMB00 intraocular lens and group C with the ZCB00 intraocular lens.The defocus curves of participants in the group A and B at three months after the surgery were collected,the modulation transfer function(MTF)values under 5 mm pupil size in all groups at three months after the surgery were collected by Topcon KR-1W vision analyzer,and the data were statistically analyzed.Results At three months after the operation,there was no significant difference in the defocus curve between group A and group B at 0D(5m)or-2.5 D~-4 D(25~40 cm),and the visual acuity of group A was better than that of group B at-1 D~-2 D(50 cm~1 m).There was no significant difference in MTF among the three groups at 15 c/d and 75 c/d at three months after the operation(P>0.05).No significant difference in MTF were observed between group A and C at 30 c/d and 45 c/d(P<0.05),but the MTF was significantly different in group B(P<0.05).While at 60 c/d,there was no significant difference between group A and B(P>0.05),but significant difference was observed in group C(P<0.05).Conclusions Compared with ZMB00,ZXR00 can provide better all-range visual acuity and visual quality for patients,but the visual quality of distance vision in a dark environment is still inferior compared with monofocal intraocular lens.
