摘  要:钠-葡萄糖共转运蛋白2抑制剂(sodium glucose co-transporter 2 inhibitor,SGLT2i)不仅可以降低血糖,还显示出心血管和肾脏保护作用。然而SGLT2i使心血管和肾脏获益的机制尚不明确,可能是多种机制共同作用的结果。SGLT2i可能通过改善血流动力学、抑制钠-氢交换器、改善心肌能量代谢方式、抑制心室重构、调节神经激素等方式,发挥心血管保护作用;通过管球反馈、调控肾脏能量代谢、抑制炎症反应、糖-盐-水效应、抑制缺氧诱导因子和钠-氢交换器等方式,发挥肾脏保护作用。本文总结了SGLT2i对心血管和肾脏的作用机制。Sodium glucose co-transporter 2 inhibitor(SGLT2i)could not only lower glucose level,but also exhibit cardiovascular and renal protection.However,the mechanism that cardiovascular and renal benefits of SGLT2i is still unclear,which might be the synergistic effects of various mechanisms.The cardiovascular protection of SGLT2i might be exerted through the access of improving hemodynamics,inhibiting sodium-hydrogen exchanger,improving myocardial energy metabolism,inhibiting ventricular remodeling as well as regulating neurohormones.Its renal protection might be activated by means of tubuloglomerular feedback,the regulation of renal energy metabolism,the inhibition of inflammatory response,the sugar-salt-water effect and the inhibition of hypoxia-inducible factor and sodium-hydrogen exchanger.The paper summarizes the related mechanisms of SGLT2i involving the cardiovascular system and kidney.
