摘　　要：目的:探讨抗凝药师在门诊患者华法林用药管理中的作用。方法:分析门诊患者华法林用药管理的典型病例,总结抗凝药师参与华法林用药管理的药学服务切入点。结果和结论:抗凝药师运用药学专业知识,在门诊患者华法林抗凝治疗中开展用药管理,为优化抗凝治疗方案提供专业建议,有利于提高华法林治疗的安全性和有效性。Objective:To explore the role of anticoagulant pharmacists in the management of warfarin medication in treatment of outpatients.Methods:Typical cases of warfarin medication management in outpatients were analyzed,and the entry points of pharmaceutical care by anticoagulant pharmacists for management of warfarin medication were summarized.Results and Conclusion:The anticoagulant pharmacists were involved in warfarin anticoagulation management in treatment of outpatients by their professional knowledge of pharmacy,and provided professional recommendations for optimization of the anticoagulation treatment regimen,which was beneficial to further improvement of clinical safety and efficacy in warfarin treatment.
