摘  要:2021年6月,海南省白沙黎族自治县一村民被诊断为布鲁氏菌病。为查明畜间疫情情况,及时控制疫情,通过现场走访、实地采样、实验室检测等方式开展了紧急流行病学调查。调查发现,该村共有羊养殖户4户,调查时共计存栏120只。对该村的4户养殖户以及1户流行病学关联养殖场户采集血清进行检测,结果均发现布鲁氏菌病阳性。对村养羊户主及密切接触者采集血清20人份进行布鲁氏菌抗体检测,结果9人确诊为阳性。病原分离鉴定发现,分离菌株为羊种布鲁氏菌3型。经与布鲁氏菌MLVA国际数据库的菌株信息比对发现,该菌株的MLVA8、MLVA11基因型为我国北方地区和中亚地区羊种布鲁氏菌常见基因型,而该菌的MLVA16基因型在数据库中没有匹配的菌株信息,说明该基因型为新发现的基因型。调查表明,此起疫情病原由外地传入的可能性较大,并有在本地定殖趋势,人畜再次感染风险较大。因此,海南省需加强动物检疫监管,采取多项防控措施,开展综合防控,消除人畜间布鲁氏菌病疫情,保障海南自由贸易港的公共卫生安全。In June 2021,a farmer was confirmed to be infected with brucellosis in Baisha Li Autonomous County of Hainan Province.In order to identify the source and prevent the disease in time,an emergency investigation was carried out by means of field visit, sampling,laboratory testing,etc.It was investigated that there were four households with a total inventory of 120 sheep as of the date of investigation.Serum samples were collected from sheep in the four households and one epidemiological related household,all of which were detected brucellosis-positive.20 serum samples were collected from the farmers and close contacts for detection of antibodies against brucellosis, and nine persons were confirmed to be positive.It was found that,by isolation and identification of the pathogen,the isolated strain belonged to Brucella melitensis type 3.Compared to the strains registered in international database of Brucella MLVA,MLVA8 and MLVA11 genotypes of the isolated strain were common in Northern China and Central Asia,but no strain matching with MLVA16 was found,indicating that the genotype was firstly found.It was investigated that the outbreak was most likely introduced from other regions,and trended to locally establish with high risk of re-infection in human and animals.Therefore,brucellosis in human and animals should be eradicated in Hainan Province through strengthening quarantine and supervision for animals and implementing comprehensive prevention and control measures to ensure public health safety of free trade port in Hainan.
