经口腔与经颏下超声引导口腔病变穿刺活检对比研究

摘　　要：目的探讨超声引导下经口腔内与经颏下粗针穿刺活检在口腔病变诊断中的安全性及临床价值。方法选取66例临床拟行口腔病变穿刺活检患者随机分为两组,超声引导下经口内粗针穿刺活检组33例,经颏下穿刺活检组33例,以手术病理结果及随访作为金标准,比较超声引导下经口腔内与经颏下粗针穿刺活检诊断口腔肿瘤的优势与不足。结果口腔组中2例,因张口受限改为经颏下穿刺活检,取材成功率为93.9%(31/33),诊断准确率和假阴性率分别为93.5%和6.5%;经颏下组7例,因距离远无法显示或无安全穿刺路径等,改为经口腔进行穿刺活检,经颏下取材成功率为78.8%(26/33),诊断准确率和假阴性率分别为88.5%和11.5%。经颏下组6例患者穿刺术后出现颌下腺炎。结论超声引导下口腔疾病穿刺活检应根据肿瘤部位选择最佳的穿刺方式,对于无张口障碍患者,经口腔内粗针穿刺活检较经颏下粗针穿刺活检更安全。Objective To compare the safety and value of transoral ultrasound-guided biopsy with trans-submental ultrasound-guided biopsy for oral lesions. Methods Sixty-six patients who were clinically proposed to undergo oral biopsy for oral lesions were randomly divided into two groups.33 cases in the ultrasound-guided transoral biopsy group and 33 cases in the trans-submental biopsy group. Taking the surgical pathological results and follow-up as the gold standard, comparing the advantages and disadvantages of ultrasound-guided transoral and trans-submental biopsy for the diagnosis of oral tumors. Results In the oral group, 2 cases were changed to submental biopsy due to restricted mouth opening. The success rate of transoral ultrasound-guided biopsy was 93.9%(31/33), and the diagnostic accuracy and false negative rates were 93.5% and 6.5%;In the submental group, 7 cases were unable to display due to the long distance or lack of safety puncture path, etc., they were changed to oral biopsy. The success rate of trans-submental ultrasound-guided biopsy was 78.8%(26/33), the diagnostic accuracy rate and false negative rates were 88.5% and 11.5%. Six patients in the submental group developed submandibular gland inflammation after puncture. Conclusions In ultrasound-guided oral biopsy, the best biopsy method should be selected according to the tumor site. For patients without mouth opening disorder, intraoral core needle biopsy is safer than trans-submental core needle biopsy.
