摘 要:目的探讨乳腺导管内乳头状瘤超声特征及误诊相关因素。方法对比分析经术后病理证实为乳腺导管内乳头状瘤(IDP)87个及乳腺导管原位癌(DCIS)107个的声像图特征,探讨误诊原因。结果 IDP组与DCIS组在肿块最大径、形态、边缘、回声类型、后方回声、钙化、血流分布等方面差异均有统计学意义(P<0.05)。Ⅳ型IDP误诊率最高。本组IDP超声诊断准确率为43.7%(38/87),误诊率为56.3%(49/87),其中误诊为乳腺恶性肿瘤28个(32.2%),4B类9个、4C类7个、5类12个。结论 IDP的超声表现多样,提高对IDP声像表现及其与病理组织学特征关系的认识,有助于提高IDP的超声诊断准确率。Objective To investigate the ultrasonographic features and misdiagnosis of intraductal papilloma(IDP) of the breast. Methods The ultrasonographic features of 87 cases of IDP and 107 cases of DCIS confirmed by postoperative pathology were analyzed to explore the causes of misdiagnosis.Results There were statistically significant difference between IDP group and DCIS group in the maximum diameter, shape, edge, echo type, posterior echo, calcification and blood flow distribution of mass(P<0.05). Type Ⅳ had the highest misdiagnosis rates. The accurate rate of ultrasonic diagnosis of IDP was 43.7%(38/87),and the misdiagnosis rate was 56.3%(49/87). Among them, 32.2%(28/87) were misdiagnosed as breast malignant tumor, including 9 cases of 4 B,7 cases of 4 C and 12 cases of category 5.Conclusions The ultrasonic features of IDP are varied. The accuracy of ultrasonic detection of IDP could be raised by improving the understanding of the ultrasonic features and the relationship between the sonographic manifestations and the pathological characteristics of IDP.
