摘 要:目的探讨单脐动脉与染色体异常的相关性,为产前咨询和临床的进一步处理提供依据。方法收集我院经彩色多普勒超声检查诊断为SUA的80例胎儿,同时选取同期超声诊断正常的非SUA胎儿220例,回顾分析SUA胎儿和非SUA胎儿的超声诊断结果、染色体结果。结果 220例非SUA胎儿中检测出染色体异常者10例,异常率4.5%;80例SUA胎儿中检测出染色体异常者9例,异常率为11.3%,其中以18-三体综合征最常见。SUA组患者染色体异常发生率高于非SUA组患者,两组相比,差异有统计学意义(P<0.05)。35例ISUA中检测出染色体异常者3例,染色体异常率8.0%。29例合并其他软指标组中检测出染色体异常者3例,染色体异常率10.3%。16例合并其他畸形组中检测出染色体异常者3例,染色体异常率18.7%。合并软指标组心室强回声斑最常见(62.1%),合并畸形组中心血管系统畸形最常见(75.0%)。结论单脐动脉合并畸形以心血管系统畸形最常见,单脐动脉增加染色体异常的风险,且以18-三体综合征最为常见,当单脐动脉胎儿合并其他软指标异常及合并畸形时,染色体异常率明显高于ISUA。Objective To explore the correlation between single umbilical artery and chromosomal abnormality, and to provide the basis for prenatal consultation and further clinical treatment. Methods A total of 80 fetuses diagnosed with SUA by color Doppler ultrasonography in our hospital were collected, and 220 fetuses without SUA were selected as normal fetuses with ultrasound diagnosis during the same period. The ultrasound diagnosis results and chromosomal results of SUA fetuses and non-SUA fetuses were retrospectively analyzed. Results Chromosomal abnormalities were detected in 10 of 220 non-SUA fetuses, the abnormal rate was 4.5%.Chromosomal abnormalities were detected in 9 of the 80 SUA fetuses(11.3%), among which trisomy 18 syndrome was the most common. The incidence of chromosomal abnormalities in SUA group was higher than that in non-SUA group, and the difference between the two groups was statistically significant(P<0.05).Chromosomal abnormalities were detected in 3 of the 35 cases of ISUA, and the rate of chromosomal abnormalities was 8.0%.Chromosomal abnormalities were detected in 3 of the 29 patients combined with other soft indexes, and the chromosome abnormality rate was 10.3%.Chromosomal abnormalities were detected in 3 of the 16 patients with other malformations, and the rate of chromosomal abnormalities was 18.7%.Ventricular strong echo spot was the most common(62.1%) in the combined soft index group, and central vascular system malformation was the most common(75.0%) in the combined malformation group. Conclusions Cardiovascular malformation is the most common complication of single umbilical artery. Single umbilical artery increases the risk of chromosomal abnormalities, and trisomy 18 syndrome is the most common one. When single umbilical artery fetuses are associated with abnormalities of other soft indicators and combined malformation, the rate of chromosomal abnormalities is significantly higher than that of ISUA.
