减压及非融合动态稳定术治疗椎管狭窄伴退行性腰椎侧凸的临床疗效研究

摘　　要：目的:探讨减压及非融合动态稳定术治疗椎管狭窄伴退行性腰椎侧凸的临床疗效。方法:2018年1月-2020年2月收治椎管狭窄伴退行性腰椎侧凸患者30例,采用腰后路有限减压、固定、非融合动态稳定术治疗,观察治疗效果。结果:本组患者平均手术时间为(195.89±26.72)min,术中出血量为(413.61±35.28)min、卧床时间为(3.51±1.28)d、住院时间为(19.78±3.27)d。患者术后疼痛视觉模拟量表(VAS)评分及Oswestry功能障碍指数(ODI)低于术前,日本骨科协会评估治疗(JOA)评分高于术前,差异均有统计学意义(P<0.05)。结论:腰后路有限减压、固定、非融合动态稳定术是治疗椎管狭窄伴退行性腰椎侧凸的有效方法。Objective:To investigate the clinical effect of decompression and non fusion dynamic stabilization in the treatment of spinal stenosis with degenerative lumbar scoliosis.Methods:From January 2018 to February 2020,30 patients with spinal stenosis and degenerative lumbar scoliosis were selected.They were treated with limited posterior lumbar decompression,fixation and non fusion dynamic stabilization.We observed the treatment effect.Results:The average operation time was(195.89±26.72)min,the amount of intraoperative bleeding was(413.61±35.28)min,the bed rest time was(3.51±1.28)days,and the hospital stay was(19.78±3.27)days.The postoperative VAS score,postoperative VAS score and ODI score were significantly lower than those before operation,and the JOA score was higher than that before operation,the difference was statistically significant(P<0.05).Conclusion:Limited posterior lumbar decompression,fixation and non fusion dynamic stabilization is an effective method for the treatment of spinal stenosis with degenerative lumbar scoliosis.
