极重度少精子症冻融精子行卵泡浆内单精子注射结局的分析

摘　　要：目的:分析极重度少精子症冻融精子行卵泡浆内单精子注射(ICSI)的疗效。方法:2020年11—12月回顾105例极重度少精子症(精子数量<1×10^(6)/mL)夫妇和61例梗阻性无精子症(OA)夫妇行ICSI的临床资料。其中47例极重度少精子症行冻融精子ICSI,58例行射出新鲜精子ICSI,61例OA患者行睾丸活检冻融精子ICSI。比较三组临床疗效。结果:三组女方年龄、不孕年限、E值、内膜厚度、获卵数及移植胚胎数比较,差异无统计学意义(P>0.05)。三组正常受精率、卵裂率、优质胚胎率、临床妊娠率、异位妊娠率及流产率比较,差异无统计学意义(P>0.05)。结论:极重度少精子症冷冻后复苏精子行ICSI,是一种有效的治疗方法,可降低手术取精的概率,其冷冻复苏方法仍有待进一步改进。Objective:To analyze the effect of intrafollicular sperm injection(ICSI)on frozen-thawed sperm in extremely severe oligospermia.Methods:To review the clinical data of 105 cases of extremely severe oligospermia(sperm count<1×10^(6)/mL)couples and 61 cases of obstructive azoospermia(OA)couples underwent ICSI from November to December 2020.Among them,47 patients with severe oligospermia underwent ICSI with frozen-thawed sperm,58 underwent ICSI with fresh ejaculated sperm,and ICSI of frozen-thawed sperm for testicular biopsy in 61 patients with OA.We compared the clinical efficacy of the three groups.Results:There were no statistically significant differences among the three groups of women's age,years of infertility,E value,intimal thickness,number of retrieved oocytes and number of embryos transferred(P>0.05).There was no statistically significant difference among the three groups of normal fertilization rate,cleavage rate,high-quality embryo rate,clinical pregnancy rate,ectopic pregnancy rate and abortion rate(P>0.05).Conclusion:In extremely severe oligospermia,ICSI is an effective method for resuscitating sperm after freezing,which can reduce the probability of sperm retrieval.The method of cryo-resuscitation still needs further improvement.
