硝普钠联合多巴胺治疗心肌梗死后左心衰竭的临床疗效及安全性评价

摘　　要：目的分析硝普钠联合多巴胺治疗心肌梗死后左心衰竭的临床疗效及安全性。方法90例心肌梗死后左心衰竭患者,采取随机数字表法分为单药组及联合组,各45例。单药组给予多巴胺治疗,观察组给予硝普钠联合多巴胺治疗。对比两组临床疗效、治疗前后心功能指标[左心室收缩末期内径(LVESD)、左心室舒张末内径(LVEDD)、左心室射血分数(LVEF)]及不良反应发生情况。结果联合组治疗总有效率95.56%高于单药组的82.22%,差异具有统计学意义(P<0.05)。治疗后,联合组LVESD(35.39±3.51)mm、LVEDD(54.23±2.97)mm小于单药组的(39.25±2.77)、(60.25±3.61)mm,LVEF(47.17±5.94)%高于单药组的(38.26±4.61)%,差异具有统计学意义(P<0.05)。两组在治疗期间均无明显不良反应。结论硝普钠联合多巴胺治疗心肌梗死后左心衰竭的临床效果显著,促进改善了患者的心功能,未增加不良反应,安全性较高。Objective To analyze the clinical efficacy and safety of sodium nitroprusside combined with dopamine in the treatment of left heart failure after myocardial infarction.Methods A total of 90 patients with left heart failure after myocardial infarction were divided into combination group and single-drug group according to random numerical table,with 45 cases in each group.The single-drug group was treated with dopamine,and the observation group was treated with sodium nitroprusside and dopamine.Both groups were compared in terms of clinical efficacy,cardiac function indexes[left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF)]before and after treatment,and occurrence of adverse reactions.Results The total effective rate of treatment was 95.56%in the combination group,which was higher than 82.22%in the single-drug group,and the difference was statistically significant(P<0.05).After treatment,LVESD(35.39±3.51)mm and LVEDD(54.23±2.97)mm of the combination group were less than(39.25±2.77)and(60.25±3.61)mm of the single-drug group,and LVEF(47.17±5.94)%was higher than(38.26±4.61)%of the single-drug group.All the differences were statistically significant(P<0.05).There were no obvious adverse reactions in the two groups during the treatment.Conclusion Combination of sodium nitroprusside and dopamine has significant effect on patients with left heart failure after myocardial infarction,and it can improve the patient’s cardiac function without increase of the adverse reactions,and is safe.
