摘 要:目的探讨新疆地区包虫病致过敏性休克患者特异性免疫反应特点。方法选择新疆医科大学第一附属医院2016年12月至2019年11月收治的肝包虫病患者30例,按照围术期是否发生过敏性休克分为对照组(未发生过敏性休克)20例,观察组(发生过敏性休克)10例,对其血清抗体水平进行测定(IgE、IgG、IgG1),应用酶联免疫吸附法(ELISA)检测血清细胞因子白细胞介素-10(IL-10)、IL-4、转化生长因子β(TGF-β)水平,采用实时荧光定量PCR(RT-qPCR)法检测外周血单个核细胞中Foxp3基因表达情况。结果观察组IgE、IgG、IgG1水平均明显高于对照组,IL-4、IL-10、TGF-β水平明显低于对照组,差异均有统计学意义(P<0.05)。观察组Foxp3 mRNA表达水平明显低于对照组,差异有统计学意义(P<0.05)。结论新疆地区包虫病所致过敏性休克患者血清IgE、IgG、IgG1水平升高,IL-4、IL-10、TGF-β水平及特异性转录因子Foxp3表达下降,引起免疫耐受状态异常,发生过敏性休克。Objective To explore the characteristics of specific immune response in patients with anaphylactic shock caused by echinococcosis in Xinjiang.Methods 30 patients with hydatidosis admitted to the First Affiliated Hospital of Xinjiang Medical University from December 2016 to November 2019 were divided into control group(without anaphylactic shock,n=20)and observation group(with anaphylactic shock,n=10)according to whether anaphylactic shock occurred during operation.The serum antibody level(IgE,IgG,IgG1)was measured and the serum cytokines interleukin-10(IL-10),IL-4 and transforming growth factorβ(TGF-β)were detected by enzyme-linked immunosorbent assay(ELISA).The expression of Foxp3 gene in peripheral blood mononuclear cells was detected by RT-qPCR.Results The levels of IgE,IgG and IgG1 in the observation group were significantly higher than those in the control group(P<0.05);IL-4,IL-10 and TGF-βlevels in the observation group were lower than those in the control group(P<0.05).The expression level of Foxp3 mRNA in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The levels of IgE,IgG and IgG1 in sera of patients with anaphylactic shock caused by echinococcosis in Xinjiang increased;the expressions of IL-4,IL-10,TGF-βlevels and specific transcription factor Foxp3 level decreased,resulting in abnormal immune tolerance and anaphylactic shock.
