摘 要:目的:分析支气管哮喘患儿GLCCI1基因rs37973位点多态性及其与糖皮质激素治疗效果、肺功能的关系。方法:选取2018年1月~2020年2月就诊于本院呼吸内科的90例支气管哮喘患儿作为研究对象,根据基因分型结果进行分组,每组均予以吸入用布地奈德混悬液治疗。观察患儿GLCCI1基因rs37973位点的多态性,比较患儿基线资料以及治疗前和治疗后肺功能指标第1秒用力呼气量(FEV_(1))、用力肺活量(FVC)、FEV_(1)占预计值百分比(FEV_(1)%pred)、当FEV_(1)下降至基线的20%时患儿累计吸入刺激物的剂量(PD20-FEV_(1)):、支气管哮喘症状积分。结果:90例患儿GLCCI1基因rs37973位点多态性表现为AA型(占28.89%)、AG型(占51.11%)、GG型(占20.00%)。3种基因型患儿性别、年龄比较无统计学差异(P>0.05)。AA型组嗜酸性粒细胞百分比高于AG型组、GG型组(P<0.05)。治疗后,3组患者FEV_(1)、FEV_(1)%pred高于治疗前,AA型组PD20-FEV_(1)高于治疗前,但低于AG型组、GG型组(P<0.05);AA型组FEV_(1)改变量、FEV_(1)%pred改变量、PD20-FEV_(1)改变量大于AG型组、GG型组(P<0.05)。治疗后,3组支气管哮喘症状积分(日间和夜间)低于治疗前(P<0.05)。结论:支气管哮喘患儿存在GLCCI1基因rs37973位点多态性,AA基因型患儿糖皮质激素治疗效果及肺功能改善程度更好,提示该基因位点多态性与支气管哮喘患儿糖皮质激素治疗效果存在一定的相关性,为糖皮质激素治疗支气管哮喘提供了参考依据。Objective:To analyze the rs37973 loci polymorphism of GLCCI1 gene in children with bronchial asthma and the relationship between the polymorphism and effects of glucocorticoid therapy and lung function.Methods:A total of 90 children with bronchial asthma treated in the respiratory department of our hospital from January 2018 to February 2020 were selected as the research subjects,and they were divided into groups according to genotyping results,and each group was treated with budesonide suspension for inhalation.The rs37973 loci polymorphism of GLCCI1 gene was observed among children.Baseline data,before and after treatment lung function indicators forced expiratory volume in one second(FEV_(1)),forced vital capacity(FVC),forced expiratory volume in one second as a percentage of the expected value(FEV_(1)%pred),cumulative dose of inhaled stimulants when FEV_(1) decreased to 20%of baseline(PD20-FEV_(1)):and bronchial asthma symptom score were compared.Results:The rs37973 loci polymorphism of GLCCI1 gene among 90 children showed AA genotype(28.89%),AG genotype(51.11%)and GG genotype(20.00%).There was no significant difference in gender and age among children with three genotypes(P>0.05).The percentage of AA group eosinophils was higher than that of AG group and GG group(P<0.05).After treatment,the FEV_(1) and FEV_(1)%pred in the three groups were higher than those before treatment,and the PD20-FEV_(1) in AA group was higher than that before treatment,but lower than that in AG group and GG group(P<0.05).The change of FEV_(1),FEV_(1)%pred and PD20-FEV_(1) in AA group were greater than those in AG group and GG group(P<0.05).After treatment,bronchial asthma symptom scores(daytime and nighttime)in the three groups were lower than that before treatment(P<0.05).Conclusion:There is a polymorphism in the rs37973 loci of GLCCI1 gene in children with bronchial asthma.The effects of glucocorticoid therapy and the improvement of lung function in children with AA genotype are more obvious,indicating that there is a certain corre
