摘  要:目的评估MRI上前交叉韧带(ACL)撕裂合并Ramp损伤的危险因素。方法回顾性分析2018年1月至2020年1月于医院接受膝关节MRI检查并在6个月内行ACL重建的218例患者的临床资料,根据关节镜结果将患者分为有Ramp损伤组和无Ramp损伤组,采用二元Logistic回归分析ACL撕裂患者发生Ramp损伤的危险因素,以评价MRI诊断Ramp损伤的价值。结果关节镜结果显示,38例(17.4%)ACL撕裂患者存在Ramp损伤。MRI诊断Ramp损伤的灵敏度为42.1%,特异度为88.3%。二元Logistic回归分析结果显示,ACL撕裂患者发生Ramp损伤的危险因素包括胫骨前移(OR=0.40,P<0.05)、股骨内侧髁骨挫伤(OR=8.96,P<0.05)。结论MRI诊断Ramp损伤的灵敏度较低;在ACL撕裂患者中,膝关节存在胫骨前移、股骨内侧髁骨挫伤时要注意观察是否合并Ramp损伤。Objective The risk factors associated with anterior cruciate ligament(ACL)tear combined with Ramp lesions in MRI were evaluated.Methods The clinical data of 218 patients undergoing MRI examination of the knee and ACL reconstruction within 6 months from January 2018 to January 2020 were retrospectively analyzed.The patients were divided into two groups,with and without Ramp lesions according to arthroscopic diagnosis.The risk factors of Ramp lesions in patients with ACL tear were analyzed by binary Logistic regression to evaluate the value of MRI in the diagnosis of Ramp lesions.Results The arthroscopic diagnosis results showed that 38 patients(17.4%)were confirmed with Ramp lesions.The sensitivity and specificity of MRI in the diagnosis of Ramp lesions were 42.1%and 88.3%,respectively.The binary Logistic regression results showed that the risk factors of Ramp lesions in patients with ACL tear were anterior shift of tibia(OR=0.40,P<0.05)and contusion of the medial femoral condyle(OR=8.96,P<0.05).Conclusion The sensitivity of MRI in the diagnosis of Ramp lesion is low.In ACL tear,care should be taken with patients who have significant risk factors for Ramp lesions,including contusion of medial femoral condyle and anterior shift of tibia.
