超声引导下经皮球囊扩张术治疗动静脉内瘘狭窄的临床效果

摘　　要：目的探讨超声引导下经皮球囊扩张术治疗动静脉内瘘狭窄的临床效果。方法选择2017年1月至2019年6月医院收治的动静脉内瘘狭窄患者64例,按照随机数字表法分为两组,各32例。对照组采用手术修补治疗,试验组采用超声引导下经皮球囊扩张术治疗,比较两组术后内瘘通畅率及并发症发生率。结果术后3个月,两组内瘘通畅率比较,差异无统计学意义(P>0.05);术后6、12个月,试验组内瘘通畅率均高于对照组,差异有统计学意义(P<0.05);试验组出血、静脉撕裂率均低于对照组,差异有统计学意义(P<0.05);两组感染率比较,差异无统计学意义(P>0.05)。结论对动静脉内瘘狭窄患者采用超声引导下球囊扩张术治疗的效果优于手术修补治疗,可有效提高术后内瘘通畅率,并减少并发症的发生,有利于改善患者预后。Objective The clinical effects of ultrasound-guided percutaneous balloon dilatation for the treatment of arteriovenous fistula stenosis patients were investigated.Methods A total of 64 patients with arteriovenous fistula stenosis admitted to the hospital from January 2017 to June 2019 were selected and divided into two groups according to the random number table method,with 32 cases in each.The control group were treated with surgical repair,while the test group were treated with ultrasound-guided percutaneous balloon dilatation.The complication rate and patency rate of postoperative internal fistula were compared between the two groups.Results There was no significant difference in the patency rate of internal fistula within 3 months after operation between the two groups(P>0.05);The patency rate of internal fistula in the test group within 6 and 12 months after operation were both higher than those in the control group,with significant differences(P<0.05);The rates of bleeding and intravenous tear in the test group were both lower than those in the control group,with significant differences(P<0.05);There was no significant difference in infection rate between the two groups(P>0.05).Conclusion The therapeutic effects of ultrasound-guided balloon dilatation for patients with arteriovenous fistula stenosis is superior to surgical repair,with higher postoperative patency rate of internal fistula,fewer complications and improved prognosis.
