摘  要:目的探讨新生儿先天性甲状腺功能低下症(CH)与出生体质量及胎龄之间的相关性。方法选取2019年6月至2020年6月在本院出生的1685例新生儿,采用时间分辨荧光免疫法开展CH筛查,对新生儿足跟血斑所对应的促甲状腺素(TSH)指标进行检测,召回TSH>9.0 mIU/L者,检查其甲状腺功能等。结果低出生体质量组、巨大儿组的CH筛查阳性率(3.59%、3.72%)高于正常体质量组(0.43%),差异有统计学意义(χ(2)=18.34、20.61,P=0.02、0.01)。超龄组新生儿CH阳性率(3.64%)高于足月组(1.42%)、早产组(1.12%),差异有统计学意义(χ(2)=4.45、7.12,P=0.03、0.02)。结论CH的发病与新生儿出生时的胎龄、体质量密切相关,巨大儿、低出生体质量儿及超龄产儿有较高的CH发病率。Objective To investigate the correlation between neonatal congenital hypothyroidism(CH)and birth weight and gestational age.Methods 1685 newborns born in our hospital from June 2019 to June 2020 were selected.Time-resolved fluorescence immunoassay was used to carry out CH screening,and the thyroid stimulating hormone(TSH)index corresponding to the neonatal heel blood spots was measured.Test,recall those with TSH>9.0 mIU/L,check their thyroid function.Results The CH screening positive rate(3.59%,3.72%)of low birth weight group and macrosomia group was higher than that of normal body weight group(0.43%),and the difference was statistically significant(χ^(2)=18.34,20.61,P=0.02,0.01).The positive rate of CH in the overage group(3.64%)was higher than that of the term group(1.42%)and the preterm group(1.12%),the difference was statistically significant(χ^(2)=4.45,7.12,P=0.03,0.02).Conclusion The incidence of CH is closely related to the gestational age and weight of newborns.Macrophages,low birth weight infants and over-age infants have higher incidence of CH.
