盆底重建联合PPH对中重度直肠脱垂患者应激反应及炎症因子的影响

摘　　要：目的探讨中重度直肠脱垂患者采用盆底重建与吻合器痔上黏膜环切术(PPH)联合治疗的效果。方法回顾性分析2005年1月至2018年12月本院收治的80例中重度直肠脱垂患者作为研究对象,将采用直肠悬吊联合肛门紧缩术治疗的40例患者作为对照组,将采用盆底重建联合PPH治疗的40例患者作为观察组。比较两组疗效、应激指标及炎症因子。结果观察组治疗总有效率为95.00%,高于对照组的77.50%(P<0.05);两组术后血清去甲肾上腺素(NE)水平、皮质醇(COR)、前列腺素E2(PGE2)水平均高于术前,差异均有统计学意义(P<0.05);术后,两组血清NE、COR、PGE2水平比较差异无统计学意义;术后,两组血清C反应蛋白(CRP)、白介素6(IL-6)、白介素10(IL-10)水平均高于术前,差异有统计学意义(P<0.05);术后,两组血清CRP、IL-6、IL-10水平比较差异无统计学意义。结论中重度直肠脱垂患者采用盆底重建与PPH联合治疗效果显著,但会对应激指标及炎症因子具有一定影响。Objective To explore the effect of pelvic floor reconstruction combined with procedure for prolapse and hemorrhoids(PPH)in the treatment of patients with moderate to severe rectal prolapse.Methods The 80 patients with moderate to severe rectal prolapse admitted to our hospital from January 2005 to December 2018 were selected as the research subjects,and the 40 patients who underwent rectal suspension combined with anal tightening were classified as the control group,the 40 patients treated with pelvic floor reconstruction and PPH were classified as the observation group.The efficacy,stress indicators and inflammatory factors between the two groups were compared.Results The total effective rate in the observation group was 95.00%,which was higher than 77.50%in the control group(P<0.05).The postoperative serum norepinephrine(NE)level,cortisol(COR),and prostaglandin E2(PGE2)levels of the two groups were higher than those before the operation,and the differences were statistically significant(P<0.05).After operation,there was no significant difference in the serum NE,COR,and PGE2 levels between the two groups.After operation,the levels of serum C-reactive protein(CRP),interleukin 6(IL-6)and interleukin 10(IL-10)in the two groups were higher than those before operation,and the difference was statistically significant(P<0.05).After operation,there was no significant difference in the serum CRP,IL-6 and IL-10 levels between the two groups were not statistically significant.Conclusion Pelvic floor reconstruction combined with procedure for prolapse and hemorrhoids in the treatment of patients with moderate to severe rectal prolapse,but at the same time it will have some effect on stress indicators and inflammatory factors.
