摘 要:目的通过观察脐血单个核细胞联合髓芯减压植骨的治疗效果,为临床早期股骨头坏死的治疗提供参考。方法对本科室2019年7月至2019年12月接收的早期股骨头坏死40例患髋,按照随机分配的原则将其分为两组,脐血单个核细胞联合髓芯减压植骨为治疗组,单纯髓心减压植骨术为对照组。通过观察患者Harris评分、VAS评分、股骨头X线整体稳定率、股骨头塌陷程度、MRI囊性变面积变化情况,评估脐血单个核细胞联合髓芯减压植骨治疗效果。结果患者治疗组Harris评分、VAS评分情况均优于对照组,差异具有统计学意义(P<0.05),股骨头X线整体稳定率良好、未见股骨头塌陷,MRI囊性变面积与对照组相比,差异均具有统计学意义(P<0.05)。结论脐血单个核细胞联合髓芯减压植骨治疗早期股骨头坏死的效果较为显著,为临床应用提供了一种更为有效的治疗方法。Objective:To observe the therapeutic effect of umbilical cord blood mononuclear cells combined with marrow core decompression and bone grafting,and to provide a reference for the treatment of femoral head necrosis in early clinical stage.Methods:Forty patients with early femoral head necrosis received from July 2019 to December 2019 in this department were divided into two groups according to the principle of random distribution.By observing patients'Harris score,VAS score,overall stability of femoral head X-ray,degree of femoral head collapse,and changes of MRI cystic variable area,the effect of umbilical cord blood mononuclear cells combined with core decompression and bone grafting was evaluated.Results:the Harris score and VAS score in the treatment group were better than those in the control group(P<0.05),the difference was statistically significant,the overall stability of femoral head X-ray was good,no femoral head collapse was observed,and the difference in MRI cystic change area was statistically significant compared with the control group(P<0.05).Conclusion:mononuclear cells of umbilical cord blood combined with core decompression and bone grafting are effective in the treatment of early femoral head necrosis,which provides a more effective treatment for clinical application.
