摘 要:随机收集54株产超广谱β内酰胺酶(ESBLs)和24株对头孢菌素类抗生素敏感的肺炎克雷伯菌,通过PCR筛选ESBLs基因型,细菌与含有一定浓度的抗生素共同孵育一定时间后,通过基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)检测抗生素水解产物的特征峰。头孢噻肟与头孢曲松水解试验分别在反应1 h与2 h后,能够很好地鉴别产ESBLs的肺炎克雷伯菌;与ESBLs基因型检测相比,特异性为100%,敏感性为96.3%。MALDI-TOF MS在临床上能够快速准确鉴定病原菌,并且在细菌耐药性检测方面具有巨大潜力,MALDI-TOF MS酶解法能够快速检测产ESBLs肺炎克雷伯菌。Fifty-four extended-spectrumβ-lactamases(ESBLs)producing strains of Klebsiella pneumoniae and 24 cephalosporins-sensitive strains were randomly collected.The mass peaks of hydrolyzed antibiotics were detected by MALDI-TOF MS after co-incubation with a certain concentration of antibiotics.The hydrolysis assay of cefotaxime and ceftriaxone was able to identify ESBLs-producing Klebsiella pneumoniae after co-incubation for 1 h and 2 h respectively.Compared with genotype detection of ESBLs,the specificity was 100%and the sensitivity was 96.3%.MALDI-TOF MS can quickly and accurately identify pathogens in clinical practice,which has great potential in the detection of bacterial resistance.The antibiotic hydrolysis assay based on MALDI-TOF MS can detect ESBLs-producing Klebsiella pneumoniae within few hours.
