安神定志汤联合艾司西酞普兰治疗阵发性室上性心动过速合并焦虑症临床研究  

Anshen Dingzhi Decoction combined with Escitalopram in the treatment of Paroxysmal Supraventricular Tachycardia with Anxiety

摘　　要：目的:探讨安神定志汤联合艾司西酞普兰治疗阵发性室上性心动过速(PSVT)合并焦虑症的疗效。方法:选取85例PSVT伴焦虑症患者,随机分成两组,对照组40例在常规治疗基础上予以艾司西酞普兰治疗,研究组45例在对照组治疗基础上加用安神定志汤(夜交藤、龙齿各40 g,茯神、炒酸枣仁各30 g,党参18 g,茯苓15 g,石菖蒲、远志各10 g)治疗,4周为1个疗程,治疗3个疗程,观察两组焦虑症状、实验室指标及复律情况。结果:研究组临床总有效率为91.11%,对照组为77.50%,两组疗效比较差异有统计学意义(P<0.01);两组治疗后血浆5-HT、NE、NT-proBNP、CK-MB水平均较治疗前降低(P<0.05),且研究组较对照组改善明显(P<0.05);研究组复律时间较对照组明显缩短(P<0.05),且复律时间分布有显著性差异(P<0.05),但两组成功复律率无显著性差异(P>0.05)。结论:安神定志汤联合艾司西酞普兰治疗PSVT合并焦虑症能减轻患者焦虑程度,改善心功能,有助于缩短复律时间。Objective:To study on the effect of Anshen Dingzhi Decoction combined with escitalopram in the treat-ment of paroxysmal supraventricular tachycardia(PSVT)with anxiety.Methods:Eight-five patients with PSVT with anxiety disorder were selected and randomly divided into two groups.In the control group 40 cases were treated with es-citalopram besides conventional treatment,and 45 cases in the study group were treated extra with Anshen Dingzhi De-coction except the treatment in the control group(Yejiaoteng 40 g,Longya 40 g,Fushen 30 g,fried Suanzaoren 30 g,Dangshen 18 g,Fuling 15 g,Shichangpu 10 g,Yuanzhi 10 g).A course of treatment was consist of 4 weeks,after 3 courses the anxiety symptoms,laboratory indicators and cardioversion of the two groups were observed.Results:The to-tal clinical effective rate of the study group was 91.11%and that of the control group was 77.50%.There was a statisti-cally significant difference in efficacy between the two groups(P<0.01);plasma 5-HT,NE,NT-proBNP,CK-MB level was lower than before treatment(P<0.05),and the study group was significantly better than the control group(P<0.05);in the study group cardioversion time was significantly shorter than that of the control group(P<0.05),and the cardio-version time distribution was significant sexual difference(P<0.05),but there was no significant difference in the suc-cess rate of cardioversion between the two groups(P>0.05).Conclusion:Anshen Dingzhi Decoction combined with es-citalopram in the treatment of PSVT with anxiety can reduce the anxiety of patients,improve cardiac function,and helps to shorten the time of cardioversion.
