摘　　要：目的探讨经椎旁肌间隙入路椎弓根螺钉系统对老年单节段胸腰椎骨折患者术后恢复效果的影响。方法回顾性分析2018年6月至2019年6月在河南圣德医院接受治疗的76例老年单节段胸腰椎骨折患者的临床资料,根据其入路方式分为A组(经椎旁肌间隙入路)38例和B组(传统入路)38例。对比分析两组矫形程度、胸腰椎功能和脊柱功能恢复效果、术后并发症发生情况。结果术后6个月,A组胸椎后凸角度、矢状面平衡、颌眉角均低于B组,A组JOA评分高于B组,但ODI评分低于B组(P<0.05)。A组并发症发生率较B组低(P<0.05)。结论经椎旁肌间隙入路椎弓根螺钉系统对老年单节段胸腰椎骨折矫形具有良好效果,可促进患者胸腰椎功能及脊柱功能恢复,降低并发症发生率。Objective To investigate the effect of pedicle screw system through paraspinal intermuscular space on postoperative recovery in elderly patients with single-level thoracolumbar fracture.Methods The clinical data of 76 elderly patients with single level thoracolumbar fractures who were treated in Shengde hospital from June 2018 to June 2019 were retrospectively analyzed,according to the approach,38 cases were divided into group A(through the paravertebral space approach)and group B(through the traditional approach).The degree of correction,the recovery of thoracolumbar function and spinal function,and postoperative complications were compared between the two groups.Results At 6 months after operation,the thoracic kyphosis angle,sagittal balance and jaw-brow angle in group A were lower than those in group B,JOA score in group A was higher than that in group B,but ODI score was lower than that in group B(P<0.05).The complication rate of group A was lower than that of group B(P<0.05).Conclusion The pedicle screw system through the paravertebral muscle interspace approach is effective in the treatment of single level thoracolumbar fracture in the elderly.It can promote the recovery of thoracolumbar function and spinal function,and reduce the incidence of complications.
