摘　　要：目的评价隧道技术联合荷包缝合在动物内镜胃成形术中的有效性及安全性,为临床研究提供参考。方法通过喂食高脂高热量饲料喂养及静脉注射小剂量链脲佐菌素构建实验性比格犬肥胖症并2型糖尿病模型。应用随机数字法将18只肥胖症并2型糖尿病比格犬按体重配伍随机分配至实验组(n=9)及对照组(n=9),实验组应用隧道技术联合荷包缝合在尼龙绳及钛夹辅助下缩小胃腔,对照组不做干预处理。记录实验组手术操作时间、术中出血量及术后并发症。比较两组比格犬术后1、3、6、12个月体重、空腹血糖、总胆固醇及甘油三脂差异。结果实验组9只比格犬均顺利完成隧道技术联合荷包缝合主导下的经内镜减重手术,操作时间为(68.5±11.3)min,术中平均出血量为(35.5±12.3)mL。其中1只比格犬发生消化道出血,经内镜止血后恢复。实验组比格犬术后体重、空腹血糖、总胆固醇及甘油三脂持续平稳下降,与术前比较均具有统计学意义(P<0.05)。而对照组呈持续上升状态。实验组比格犬术后1、3、6、12个月体重、空腹血糖、总胆固醇及甘油三脂均较对照组低,差异比较具有统计学意义(P<0.05)。结论隧道技术及荷包缝合技术应用于内镜减重手术中可充分发挥其创伤小、可逆性、可重复性等优势,且短期内体重、血糖及血脂等指标均呈下降趋势,并发症较少,安全性较高。Objective To evaluate the effectiveness and safety of tunnel technique combined with purse-string suture in animal endoscopic gastroplasty and further provide reference for clinical research.Methods The experimental beagle dog obesity and type 2 diabetes model was constructed by feeding high-fat and high-calorie feed and intravenous injection of small dose of streptozotocin.Using the random number method,a total of 18 obese Beagle dogs with type 2 diabetes was randomly assigned to the experimental group(n=9)and the control group(n=9)according to their weight compatibility.The experimental group used tunnel technique combined with purse string suture with the aid of nylon rope and titanium clip to reduce the gastric cavity.There was no intervention in the control group.The operation time,intraoperative blood loss and postoperative complications of the experimental group were recorded.The differences in weight,fasting blood glucose,total cholesterol and triglyceride values of the two groups of Beagle dogs 1,3,6,and 12 months after surgery were compared.Results The 9 Beagle dogs in the experimental group successfully completed the oral gastroplasty led by tunnel technique combined with purse-string suture.The average operation time was(68.5±11.3)min,and the average intraoperative blood loss was(35.5±12.3)mL.One of the Beagles had gastrointestinal bleeding and recovered after hemostasis by endoscopy.The body weight,fasting blood glucose,total cholesterol and triglyceride values in the experimental group of beagle dogs continued to decrease steadily after surgery,which were statistically significant compared with those before surgery(P<0.05).The values in the control group continued to rise.Beagle dogs in the experimental group had lower body weight,fasting blood glucose,total cholesterol and triglyceride values 1,3,6,and 12 months postoperatively than those in the control group.Conclusion The application of tunnel technique and purse-string suture technique in endoscopic bariatric surgery can give full play to its
