摘　　要：目的探讨中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)与无家族史的2型糖尿病肾病(DN)的相关性。方法纳入无相关家族史的2型糖尿病(T2DM)患者295例,根据尿微量白蛋白/肌酐水平,将T2DM患者分为正常白蛋白尿组110例、微量白蛋白尿组90例、大量白蛋白尿组95例,同时随机抽取本院健康体检者99例作为对照,分析NLR、PLR在各组的差异及与DN严重程度的相关性。结果①与对照组相比,正常白蛋白尿组、微量白蛋白尿组、大量白蛋白尿组NLR水平明显增高(P<0.05),其中,NLR水平在DM组间差异有统计学意义(P<0.05),而PLR水平在各组间无统计学差异(P>0.05);②Pearson相关性分析显示尿微量白蛋白/肌酐与糖尿病病程、HbA1c、Scr、TC、LDL-C、中性粒细胞、NLR呈正相关;③Logistic回归分析显示,HBA1C、Scr、LDL-C、中性粒细胞、NLR为糖尿病肾病的独立影响因素;④ROC曲线结果显示:NLR对DN具有较高的诊断价值,当NLR≥2.567时,对糖尿病肾病的诊断价值最大。结论NLR是2型糖尿病肾病的独立危险因素之一,可作为无家族史的2型糖尿病肾病患者的早期预测指标。Objective To investigate the correlation between neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and type 2 diabetic nephropathy(DN)without family history.Methods A total of 295 type 2 diabetes mellitus(T2DM)patients without family history were included.According to the level of microalbuminuria/creatinine,T2DM patients were divided into normal albuminuria group(110 cases),microalbuminuria group(90 cases)and massive albuminuria group(95 cases).At the same time,a total of 99 healthy people in our hospital were randomly selected as controls.The differences of NLR and PLR in each group and the correlation with the severity of DN were analyzed.Results①Compared with the control group,NLR levels in the normal albuminuria group,microalbuminuria group and massive albuminuria group were higher,in which NLR levels were different between the DM group,while PLR levels were not significantly different among the groups;②Pearson correlation analysis showed that microalbuminuria/creatinine was positively correlated with diabetes duration,HbA1c,Scr,TC,LDL-C,neutrophils and NLR;③Logistic regression analysis showed that HBA1c,Scr,LDL-C,neutrophils and NLR were independent influencing factors of diabetic nephropathy;④ROC curve results showed that NLR had high diagnostic value for DN.When NLR≥2.567,the diagnostic value for diabetic nephropathy was the highest.Conclusion NLR is one of the independent risk factors for type 2 diabetic nephropathy and could be used as an early predictor of nephropathy in patients with type 2 diabetes without a family history.
