乳管镜在病理性乳头溢液中的诊断价值

摘　　要：目的探讨乳管镜在病理性乳头溢液中的诊断价值。方法回顾性分析北京市通州妇幼保健院2018年8月~2019年12月收治的120例病理性乳头溢液患者的临床资料,分析患者临床特征、乳管镜检查结果与术后病理的关系,比较乳管镜与超声对导管内病灶的检出率和诊断符合率情况。结果位于中央(Ⅰ~Ⅱ级)乳管、质硬及易出血的导管内病灶术后病理为导管内乳头状瘤概率较其他良性病变高,差异具有统计学意义(P<0.05);血性溢液、乳管壁毛糙、肿瘤质地硬及肿瘤直径大的患者患恶性肿瘤风险较良性肿瘤高,差异具有统计学意义(P<0.05);乳管镜和超声检出率分别为93.2%,36.1%,乳管镜检出率高于超声,差异有统计学意义(P<0.05)。结论乳管镜检查对病理性乳头溢液诊断和治疗有重要价值。Objective To study the diagnostic value ductoscopy in patients with pathologic nipple discharge.Methods Retrospectively analyzed the clinical data of 120 cases of pathologic nipple discharge that were hospitalized in the Beijing Tongzhou District Maternal and Child Health Hospital from August 2018 to December 2019.To analyze the relationship between clinical features,ductoscopy and postoperative pathology.To analyze and compare the detection rate and diagnosis coincidence rate of pathological nipple discharge lesions by ultrasonography and ductoscopy.Results The intraductal lesions located in the central(grade I-II)breast duct,hard and easy to bleed are more likely to be intraductal papilloma,the difference was statistically significant(P<0.05).Patients with bloody discharge,rough breast wall,hard tumor texture and large tumor diameter have higher risk of malignant tumor,the difference was statistically significant(P<0.05).The detection rates of ductoscopy and ultrasound were 93.2%and 36.1%,respectively,the difference was statistically significant(P<0.05).Conclusion Ductoscopy is of great value in the diagnosis and treatment of pathological nipple discharge.
