男性乳腺发育症患者行乳腔镜腋窝入路手术治疗的效果观察

摘　　要：目的:探讨男性乳腺发育症行乳腔镜腋窝入路手术的临床效果。方法:回顾分析2015年1月至2019年12月诊断为乳腺发育症的90例男性患者的临床资料,将90例患者分为对照组(n=45)与研究组(n=45),其中对照组行传统手术,研究组行乳腔镜腋窝入路手术,对比分析两组治疗效果。结果:两组单侧手术时间、术中失血量、拔管时间、住院总时间差异无统计学意义(P>0.05)。术后6个月,研究组的患者整体自我满意度、手术区域皮肤满意度、双侧乳房对称性满意度、乳头感觉满意度评分均高于对照组,差异有统计学意义(P<0.05)。两组术前人际关系敏感、强迫症状、躯体化、抑郁、焦虑五方面心理状况评分差异无统计学意义(P>0.05);治疗后,研究组评分低于对照组,差异有统计学意义(P<0.05)。结论:男性乳腺发育症患者行乳腔镜腋窝入路手术更利于维护外形美观,患者接受程度更高,值得临床推广。Objective:To investigate the clinical effect of endoscopic operation in axillary approach for the treatment of gynecomastia.Methods:From Jan.2015 to Dec.2019,90 male patients with gynecomastia were selected and divided into control group(n=45)and study group(n=45).The control group was treated with traditional surgery,and the study group was treated with endoscopic operation in axillary approach,the therapeutic effects of the two groups were retrospectively analyzed and compared.Results:There was no significant difference in operation time of unilateral treatment,intraoperative blood loss,extubation time and total hospital stay between the two groups(P>0.05).Six months after operation,the scores of overall self-satisfaction,skin satisfaction of operation area,bilateral breast symmetry satisfaction and nipple sensation satisfaction of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in psychological status score of interpersonal sensitivity,obsessive-compulsive symptoms,somatization,depression and anxiety between the two groups before surgery(P>0.05).After treatment,the scores of the study group were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusions:Compared with the traditional operation with areola incision,endoscopic operation in axillary approach is more conducive to maintenance of the appearance in male patients with gynecomastia,it is more acceptable to patients and worthy of clinical promotion.
