针灸治疗犬胸腰椎间盘疾病的现状

摘　　要：胸腰椎间盘疾病(intervertebral disc disease,IVDD)是犬常见的脊髓功能障碍病因,近年来小动物临床上针灸治疗此病症的相关研究逐渐丰富。论文依据近年研究成果,对犬胸腰椎间盘疾病的中兽医学病因、针灸治疗方法、疗效及原理进行综述。根据中兽医理论,IVDD病因可概括为肾气不足及外邪入侵导致的气血瘀滞、后肢不用。在治疗效果方面,与西药和外科手术治疗相比,针灸治疗均展现优势。针灸对于该病症具有镇痛、消炎、缓解神经根压迫、促进组织修复与再生的作用,其机理有多因素相关性。不同文献的治疗方案存在一定差异,但大多针灸处方固定,缺少随证调方。笔者认为在进一步研究时可尝试建立犬IVDD中兽医辨证分型及对应的针灸治疗方案,达到方证对应,从而更规范地指导临床实践。Thoracolumbar intervertebral disc disease(IVDD)is a common cause of spinal cord dysfunction in canine.In recent years,research on acupuncture treatment of this disease has gradually enriched.Based on the existing research results,this paper summarized the etiology of traditional Chinese medicine(TCM)theory,acupuncture treatment and its effect and mechanism of thoracolumbar IVDD in canine.According to the TCM theory,the disease is due to kidney-Qi deficiency and invasion of external pathogens,which causing the stagnation of Qi and blood and paralysis of the hind limbs.Compared with western medical treatment and decompressive surgery,acupuncture treatment has its advantages in the treatment effect.Acupuncture can achieve analgesia,anti-inflammatory,relieve nerve root compression,expedite tissue reparation and regeneration.And its mechanism is related to multiple factors.There are some differences in specific treatment among the literatures,but most of them use fixed prescriptions,and have not mention adjust according to symptoms.We think it is necessary to formulate different acupuncture prescription according to syndrome differentiation of traditional Chinese veterinary medicine in further research,which can guide clinical practice in a better way.
