摘　　要：目的探索上海市男男同性性行为人群(men who have sex with men, MSM)对人类免疫缺陷病毒(human immunodeficiency virus, HIV)抗体唾液快速检测试剂自检(口腔自检)方式检测HIV的使用情况和影响因素。方法 2019年1月—2020年12月,社区组织"上海心生"和上海市静安区疾病预防控制中心自愿咨询检测(voluntary counseling testing, VCT)门诊,共同招募MSM为研究对象。采用自行设计的调查问卷,在知情同意前提下,以问卷星网站在线调查的方式开展调查,收集其人口学特征、性行为特征、对口腔自检的认知和使用情况等信息。结果共2 589名MSM纳入研究,使用过口腔自检1 427例(55.12%)。多因素logistic回归分析显示,既往使用口腔自检情况在年龄<50岁(OR=1.87,95%CI:1.18~2.96),最近6个月男性性伴数≥2个(OR=1.33,95%CI:1.12~1.58),最近1年在检测点做过HIV检测(OR=1.92,95%CI:1.53~2.41),朋友/性伴中有人使用口腔自检(OR=8.49, 95%CI:5.78~12.48)或者不清楚朋友/性伴中是否使用过口腔自检(OR=1.54, 95%CI:1.04~2.48)差异有统计学意义(P<0.05)。口腔自检试剂价格从60元下降到40元、20元时弹性系数分别为2.42、1.47。结论上海市MSM口腔自检使用率相对较低,年龄、对高危因素的感知程度以及朋友/性伴推荐是影响HIV口腔自检使用的重要因素。降低试剂价格可能促进口腔自检的使用。Objective To understand the self use of oral fluid HIV rapid test among men who have sex with men(MSM) in Shanghai, and to discuss its influencing factors. Methods From January 2019 to December 2020, MSM were recruited through community based organizations(CBOs) and VCT clinics of local CDC.Under the full consent of participants, a self designed questionnaire star website based online survey was conducted to collect general demographic information, sexual behaviors, awareness and usage of self-testing. Results A total of 2 589 MSM were recruited and 55.12%(1 427/2 589) performed oral self-testing.Multivariate logistic regression analysis showed that MSM with age under 50(OR=1.87,95%CI: 1.18-2.96), having 2 or less male sexual partners in the last 6 month(OR=1.33,95%CI: 1.12-1.58), received HIV testing in the most recent year(OR=1.92, 95%CI: 1.53-2.41), having friends or sex partner who used oral quick HIV self-testing(OR=8.49, 95%CI: 5.78-12.48) or not sure whether their friends or sexual partner used oral quick HIV self-testing(OR=1.54, 95%CI: 1.04-2.48) were more likely to perform oral self-testing(P<0.05).The price of the test reagents also affect the self-testing demands, with the price dropping from 60 to 40 RMB or from 60 to 20 RMB, the elastic coefficient changed from 2.42 to 1.47.Conclusions Only around half of MSM in Shanghai perform HIV self testing using oral fluid HIV rapid test.Age,awareness of high-risk factors and friend/sexual partner recommendations are major factors affecting the use of oral quick HIV self-testing.In addition,lowering the price of test reagents may encourage the use of oral self-testing.
