急性心肌梗死预后与缺血性J波心电图特征之间的关联性研究

摘　　要：目的研究急性心肌梗死预后与缺血性J波心电图特征之间的关联性。方法回顾性分析我院功能科2018年6月~2019年12月收入的150例急性心肌梗死患者临床资料,依据患者预后分为存活组120例、死亡组30例,对两组缺血性J波心电图特征进行对比。结果两组急性心肌梗死患者主要不良心血管事件发生率、缺血性J波形态、缺血性J波振幅、缺血性J波分布导联相比较,差异有统计学意义(P<0.05);Spreaman相关性分析结果提示:切迹样改变、高缺血性J波振幅与急性心肌梗死患者预后呈负相关性(r=-0.67、-0.70,P<0.05),缺血性J波分布导联与急性心肌梗死患者预后无相关性(r=0.23,P>0.05)。结论急性心肌梗死预后与缺血性J波切迹样改变、高缺血性J波振幅呈负相关性,可作为今后临床预测其预后的重要按照指标。Objective To study the correlation between the prognosis of acute myocardial infarction and the characteristics of ischemic J wave electrocardiogram.Methods A retrospective analysis of clinical data of 150 patients with acute myocardial infarction received from the functional department of our hospital from June 2018 to December 2019 was divided into 120 survival groups and 30 death groups according to the prognosis of the patients.Wave ECG characteristics were compared.Results The incidence of major adverse cardiovascular events, ischemic J-wave morphology, ischemic J-wave amplitude, and ischemic J-wave distribution leads were compared between the two groups of patients with acute myocardial infarction.The differences were statistically significant(P<0.05);Spreaman correlation analysis results suggest that notch-like changes, high ischemic J-wave amplitude and prognosis of patients with acute myocardial infarction have a negative correlation(r=-0.67,-0.70,P<0.05),ischemic J-wave distribution There was no correlation between the lead and the prognosis of patients with acute myocardial infarction(r=0.23,P>0.05).Conclusion The prognosis of acute myocardial infarction is negatively correlated with ischemic J-wave notch-like changes and hyper-ischemic J-wave amplitude, which can be used as an important indicator for clinical prognosis.
