中期妊娠脐动脉超声血流参数及血清IL-6、TNF-α、CRP对子痫前期的二级预防价值

摘　　要：目的:评价中孕期脐动脉超声血流参数及血清炎性因子对子痫前期的预测诊断价值。方法:回顾性分析2019年1月-2020年6月本院收治的子痫前期孕妇37例(重度子痫前期17、轻度子痫前期20例)资料,同期同基线健康孕妇50例为对照组,比较各组中期妊娠脐动脉超声血流参数及血清白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平,运用ROC曲线分析上述指标对子痫前期的预测诊断价值及临界值。结果:中期妊娠时孕妇脐动脉血流搏动指数(PI)、血流阻力指数(RI)、脐动脉收缩期最大血流速度(S)与舒张期末期血流速度比及血清IL-6、TNF-α、CRP水平重度子痫前期组最高,轻度子痫前期组居中,对照组最低(P<0.05)。ROC曲线分析,PI、RI、S/D、IL-6、TNF-α和CRP均有预测价值,而血清炎性因子诊断效能相对较高,诊断临界值分别为1.117、0.784、3.902、0.470pg/ml、1.644ng/ml和6.295mg/L。结论:中期妊娠孕妇脐动脉超声血流参数PI、RI、S/D及血清IL-6、TNF-α、CRP对子痫前期均有一定预测诊断价值,提示临床对此进行关注以有效进行子痫前期的二级预防。Objective:To evaluate the value of umbilical artery ultrasound blood flow parameters by ultrasound and serum inflammatory factor levels,such as interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and C-reactive protein(CRP),of the pregnant women during the second trimester of pregnancy for predicting their preeclampsia.Methods:By retrospective analysis,37 pregnant women with preeclampsia from January 2019 to June 2020 were included and were divided 17 cases with severe preeclampsia in group A and 20 cases with mild preeclampsia in group B,and 50 healthy pregnant women during the same period were included in group C.The values of umbilical artery ultrasound blood flow parameters by ultrasound,and the serum IL-6,TNF-α,and CRP levels of the women during the second trimester of pregnancy were compared among the three groups.ROC curve analysis was used to evaluate the value of umbilical artery ultrasound blood flow parameters values by ultrasound,and the serum IL-6,TNF-α,and CRP levels for predicting preeclampsia.And the diagnostic cut-off value was calculated.Results:The ultrasonographic blood flow parameters of umbilical artery,such as pulsation index(PI),resistance index(RI),and the ratio of the maximum flow velocity in systolic period(S)to the maximum flow velocity in diastolic period(D)of umbilical artery(S/D),and the serum IL-6,TNF-α,and CRP levels of the women had significant different among the three groups,and which of the women in group A were the highest,followed by those of the women in group B,and those of the women in group C were the lowest(P<0.05).ROC curve analysis showed that the values of PI,RI,and S/D,and the levels of IL-6,TNF-α,and CRP all had predictive values,which’s diagnostic cut-off values them were 1.117,0.784,3.902,0.470pg/ml,1.644ng/ml,and 6.295mg/L,respectively.The diagnostic efficacy of serum inflammatory factors(IL-6,TNF-α,and CRP)levels had relatively higher diagnostic efficacy.Conclusion:The values of umbilical artery ultrasound blood flow parameters by ultrasound(PI,RI,and S
