1728例皮肌炎患者抗特异性肌炎谱抗体结果分析

摘　　要：目的探讨抗特异性肌炎谱抗体在皮肌炎患者中的临床意义,为临床提供辅助诊断依据。方法随机选取2019年10月至2020年10月送检该单位进行血清抗特异性肌炎谱抗体检测的1728例皮肌炎患者为研究对象,采用免疫印记法进行抗体(抗EJ抗体、抗Fibrillarin抗体、抗HA抗体、抗JO-1抗体、抗KS抗体、抗KU抗体、抗MDA5抗体、抗Mi-2抗体、抗NOR-90抗体、抗NXP2抗体、抗OJ抗体、抗PL-12抗体、抗PL-7抗体、抗PM-SCL100抗体、抗PM-SCL75抗体、抗RNA-PⅢ抗体、抗Ro-52抗体、抗SAE1抗体、抗SAE2抗体、抗Scl-70抗体、抗SRP抗体、抗Th/To抗体、抗TIF1γ抗体、抗ZO抗体)检测并分析结果。结果1728例皮肌炎患者中抗特异性肌炎谱抗体阳性总例数为907例,总阳性率为52.49%。41~<51岁阳性率最高,为62.58%;其次是61~<71岁,为57.80%;1~<18岁最低,为30.82%。男性抗特异性肌炎谱抗体阳性率为42.58%,女性阳性率为58.93%,组间比较差异有统计学意义(P<0.05)。抗Ro-52抗体总阳性率为24.83%,女性阳性率为18.17%,男性阳性率为6.66%;抗JO-1抗体总阳性率为7.29%,女性阳性率为5.27%,男性阳性率为2.03%;抗RNA-PⅢ抗体总阳性率为6.25%,女性阳性率为4.34%,男性阳性率为1.91%。单独1种抗体阳性有475例,阳性率为52.37%;其次是2种抗体阳性323例,阳性率为35.61%;3种抗体阳性80例,阳性率为8.82%。结论抗特异性肌炎谱抗体检测可为皮肌炎的早期诊断、治疗及预后判断等提供依据。Objective To explore the clinical significance of anti-specific myositis spectrum antibodies in patients with dermatomyositis,and to provide clinical evidence for auxiliary diagnosis.Methods A total of 1728 patients with dermatomyositis who were sent to the unit for serum anti-specific myositis spectrum antibody detection from October 2019 to October 2020 were randomly selected as the research objects,and the antibodies(anti-EJ antibody,anti-Fibrillarin antibody,anti-HA antibody,anti-JO-1 antibody,anti-KS antibody,anti-KU antibody,anti-MDA5 antibody,anti-Mi-2 antibody,anti-NOR-90 antibody,anti-NXP2 antibody,anti-OJ antibody,anti-PL-12 antibody,anti-PL-7 antibody,anti-PM-SCL100 antibody,anti-PM-SCL75 antibody,anti-RNA-PⅢantibody,anti-Ro-52 antibody,anti-SAE1 antibody,anti-SAE2 antibody,anti-Scl-70 antibody,anti-SRP antibody,anti-Th/To antibody,anti-TIF1γantibody,anti-ZO antibody)detection were performed by immunoblotting method and the results were analyzed.Results Among the 1728 patients with dermatomyositis,the total number of positive anti-myositis spectrum antibodies was 907,and the total positive rate was 52.49%.The positive rate of 41-<51 years old was the highest,62.58%;followed by 61-<71 years old,57.80%;1-<18 years old was the lowest,30.82%.The positive rate of male anti-specific myositis spectrum antibody was 42.58%,and the positive rate of female was 58.93%.The difference between the groups was statistically significant(P<0.05).The total positive rate of anti-Ro-52 antibody was 24.83%,the positive rate of women was 18.17%,the positive rate of men was 6.66%;the total positive rate of anti-JO-1 antibody was 7.29%,the positive rate of women was 5.27%,and the positive rate of men was 2.03%;the total positive rate of anti-RNA-PⅢantibody was 6.25%,the positive rate of women was 4.34%,and the positive rate of men was 1.91%.There were 475 cases with a single antibody positive,with a positive rate of 52.37%;followed by 323 cases with two antibodies with a positive rate of 35.61%;80 cases with three antibodi
