摘　　要：目的探究血必净注射液联合盐酸氨溴索治疗老年重症肺炎对患者血清DAO、D-Lac、ET水平及动脉血气的影响。方法选取我院呼吸内科收治的老年重症肺炎患者共118例,通过随机数字表法分为对照组和观察组,各59例。两组均行常规治疗,对照组在常规治疗基础上给予盐酸氨溴索治疗,观察组在对照组基础上给予血必净注射液治疗。两组均治疗14 d。比较治疗前后两组动脉血气、T淋巴细胞亚群、肠黏膜屏障功能及呼吸力学指标。结果治疗后,观察组SaO_(2)、PaO_(2)、全血CD^(+)_(3)、CD^(+)_(4)、CD^(+)_(4)/CD^(+)_(8)水平较治疗前均升高,观察组高于对照组(P<0.05);Pplat、PIP、mRaw、Paw、PaCO_(2)、全血CD^(+)_(8)、血清DAO、D-Lac、ET水平较治疗前均降低,观察组低于对照组(P<0.05)。结论血必净注射液联合盐酸氨溴索治疗老年重症肺炎可降低患者血清DAO、D-Lac、ET水平,改善肠黏膜屏障功能、动脉血气及呼吸力学指标,疗效确切。Objective To investigate the effect of Xuebijing injection combined with ambroxol hydrochloride in the treatment of elderly patients with severe pneumonia on serum DAO,D-Lac,ET levels and arterial blood gas.Methods A total of 118 elderly patients with severe pneumonia admitted to the department of respiratory medicine of our hospital were selected and divided into the control group and the observation group by random numbers table method,with 59 cases in each group.Both groups were treated with conventional treatment,the control group was given ambroxol hydrochloride on the basis of conventional treatment,and the observation group was given Xuebijing injection on the basis of the control group.Both groups were treated for 14 d.Arterial blood gas,T lymphocyte subsets,intestinal mucosal barrier function and respiratory mechanics indexes were compared between the two groups before and after treatment.Results After treatment,the levels of SaO_(2),PaO_(2),CD^(+)_(3),CD^(+)_(4),CD^(+)_(4)/CD^(+)_(8)in whole blood of the observation group were higher than before treatment,and the observation group were higher than the control group(P<0.05);Pplat,PIP,mRaw,Paw,PaCO_(2),whole blood CD8+,serum DAO,D-Lac and ET levels were lower than before treatment,the observation group were lower than the control group(P<0.05).Conclusion Xuebijing injection combined with ambroxol hydrochloride in the treatment of elderly patients with severe pneumonia can reduce serum DAO,D-Lac,ET levels,improve intestinal mucosal barrier function and arterial blood gas,respiratory mechanics indexes,with accurately effect.
