摘 要:目的探讨SPECT/CT核素融合技术与HRCT在脑脊液漏定位中的临床价值。方法选取我院神经外科收治的脑脊液漏患者54例,其中男性30例,女性24例,平均年龄(43.1±3.0)岁。入选患者均行HRCT和SPECT/CT融合显像。术中探查和随访结果作为金标准,与影像学检查进行比较。结果影像检查结果显示,HRCT诊断脑脊液漏口的灵敏度、特异度和准确性分别为87.14%(61/70)、75.00%(12/16)和84.88%(73/86);^(99)mTc-DTPA SPECT/CT核素示踪显像诊断脑脊液漏口的灵敏度、特异度和准确性分别为97.14%(68/70)、81.25%(13/16)和94.19%(81/86)。SPECT/CT在脑脊液漏口检出与术中探查和术后随访结果有较好的一致性(Kappa=0.80,P<0.05),HRCT对于脑脊液漏口的检出一致性一般(Kappa=0.55,P<0.05)。结论99mTc-DTPA SPECT/CT融合技术对脑脊液漏具有较高的诊断价值。Objective To evaluate the clinical value of SPECT/CT radionuclide fusion imaging and HRCT in the localization of cerebrospinal fluid leakage.Methods A total of 54 patients with suspected cerebrospinal fluid leakage treated by neurosurgery in the Northern Theater General Hospital from January 2007 to December 2015 were enrolled,including 30 males and 24 females,aged(43.1±3.0)years old.CT and SPECT/CT fusion imagings were performed on all subjects.Surgery and follow-up results were used as gold standards compared with imaging.Results The results of imaging examination showed that the sensitivity,specificity and accuracy of HRCT for the leak location of cerebrospinal fluid leakage were 87.14%(61/70),75.00%(12/16)and 84.88%(73/86),respectively and 97.14%(68/70),81.25%(13/16)and 94.19%(81/86)for ^(99)mTc-DTPA SPECT/CT.SPECT/CT was in good concordance(Kappa=0.80,P<0.05)with the detection of cerebrospinal fluid leakage compared with the surgical examination and follow-up results,while HRCT was in moderate concordance(Kappa=0.55,P<0.05).Conclusion It is of great diagnostic value to use SPECT/HRCT fusion imaging for cerebrospinal fluid leakage.
